25 | On May 22, 2026, at 9:30 AM, Licensing Program Analyst (LPA) Avelina Martinez made an unannounced visit to this facility to conduct an annual required inspection. LPA Martinez met with Gilda Giner and explained the purpose of today's visit.
LPA Martinez inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, laundry room, activity room, and outside courtyards of the facility to ensure compliance with Title 22 regulations.
Administrator holds current certificate. The facility is licensed for thirty-four non-ambulatory residents, which all may be bedridden. There are currently twenty-five residents who reside at this facility.
The LPA Martinez toured the facility with Gilda Giner on May 22, 2026, at 11:30 AM.
LPA Martinez reviewed ten resident files and ten staff files. All files were maintained current. LPA Martinez reviewed one medication administration record (MAR), and the MAR was kept current. The facility has a first aid kit. The facility has an infection control plan and a disaster plan. The last emergency drill was on April 15, 2026. The fire extinguishers were in good repair. The facility common areas, bedrooms and bathrooms were sanitary. The facility temperature measured at 71 degrees, and the water temperature measured at 110 degrees. The facility had an adequate food supply and the facility kitchen was furnished and sanitary. The exterior of the facility was clear of debris, and the resident patio areas were furnished and sanitary.
Based on this year's annual inspection, there were no citations given. An exit interview was conducted, and a copy of this report was provided to the facility.
