On 2.12.25, LPA Diaz conducted an unannounced inspection at the facility. LPA was greeted and granted access by the front office staff. Director, Alexandria Chung guided LPA on a tour of the facility. LPA explained the purpose of the visit was to deliver complaint findings for the complaint received at Palmdale RO on 11.15.24. At the time of the visit LPA observed 18 children in care with 5 staff in the Yotes classroom. LPA also observed 10 children in care with 4 staff in the Pups classroom. LPA conducted a safety inspection which yielded zero deficiencies.

During this investigation, LPA conducted confidential interviews with staff and parents and collected documents pertinent to the investigation. Evidence gathered revealed several incidents occurring between a child with special needs (C1) and other children in the classroom. Due to the center's requirement to meet both Title 5 and Title 22 requirements the center staff was limited in resources to assist C1 and the option to release the child from care.
