25 | On 03/27/2024 at 1:22 PM, Licensing Program Analyst (LPA) Raymond Moorehead conducted a case management inspection with Kim Montague, Director. A case management inspection is being conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on 03/19/2024 as the incident was self-reported.
The UIR documented an incident where a child reportedly was hit in the head with a shovel by another child. It was reported that the child hit the other child instead of using their words. Further, the hit to the head resulted in the child that was hit needing medical attention. The facility tended to the child with care immediately, and the child was safely picked up by a parent/authorized representative within 15 to 30 minutes. The child has since then returned to the facility and activity is normal.
During today's visit, LPA investigated the incident, Director and Staff were interviewed, documentation was obtained, a tour/census was taken, and the following was disclosed. It was reported that both children involved in the incident were digging holes in the sand next to each other. Then, it was stated that the child that was hit sat in the other child's hole in the sand, and the child hit the child as a response instead of using their words. Several teachers were present at the time, and assisted the child that got hit by providing gauze, bandages, and ice for the child.
LPA observed the area in which the incident took place and observed holes in the sand that were dug by children in care. LPA also observed the shovel that one child used to hit another child. The shovel was observed to be a plastic shovel that is intended for children. No safety hazards were observed at this time.
Based on the information obtained, there appeared to be no violations of Title 22 Regulations pertaining to the reported incident at this time. LPA conducted an exit interview with Kim Montague, Director and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days. |
