Licensing Program Analyst (LPA) Michelle Echeverria made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with DSP Porsha Hull and was granted entry to the facility. The facility is an Adult Residential Facility (ARF) level 4I designated home vendor by Inland Regional Center.

The facility is a (4) bedrooms (3) bathrooms home and, with a kitchen/dining area, living room, laundry area, attached garage. LPA was accompanied by Porsha Hull and Patricia Akintoye to conduct a general overall inspection, which included, but was not limited to, the following:

Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 76 degrees Fahrenheit. LPA inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting. Bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The hot water temperature tested within regulation at 120 degrees Fahrenheit. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, license, and administrator certificate were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. There was a designated space for client/staff files. LPA observed two smoking detector alarms out of service, main light in master bedroom not functioning, two bedroom window screens broken, and fire extinguisher not serviced annually. Deficiency issued.

Food Service: Non-perishable and perishable food supply is sufficient for number of clients in care. Facility has a variety of food available for clients. Dishes, cups, and utensils were also stored properly.

Yard/Outside: One shaded patio furniture for outdoor seating along with (1) shed used for storage. Side gate with self-latching handle on the right side of the house that leads into the backyard. All outdoor pathways were free of obstructions.

Record Review: LPA reviewed client files for admission agreements, updated physician reports, and needs and services plans. LPA observed all client records not updated with current physician's report, IPP and SIRs. Deficiency issued. LPA also reviewed staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. P & I funds was audited and matched with ledger. LPA observed that the facility did not have an Emergency Disaster Plan. Deficiency issued. LPA observed all pages missing from the Infection Control Plan except for the last page. Deficiency issued. LPA observed that the facility did not conduct quarterly emergency drills since 7/29/24. Deficiency issued.

Deficiencies were cited during this visit. An exit interview was conducted where this report LIC809, LIC809C, LIC809D and appeal rights were discussed and copies were provided to DSP, Porsha Hull.
