Based on the interviews and observations conducted LPA determined the allegation to be Unsubstantiated.
Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
Allegation two: Residents are not properly cared for.
Licensing Program Analyst (LPA) Paola Guerrero interviewed Five (5) staff members who all stated that facility provide pendants for residents to use when needed care or assistance in addition staff also stated that staff/caregivers carry walkie talkies to respond to residents needs when needed. LPA interviewed Five (5) residents all who stated that their needs and care are being met by caregivers and staff. Residents also stated that staff goes out of their way to attend their needs in a timely manner when residents need assistance.

Allegation three: Facility is dirty.
Licensing Program Analyst (LPA) Paola Guerrero conducted a tour of the facility and was accompanied by Facility Administrator Rebecca Parra. While on tour LPA observed facility to be in good condition. LPA inspected room 102, 107, 108, 104, and 110 to be well furnished and clear of obstruction. LPA tour dining area and activity room area all to be in good and clean condition and free of odor. LPA interviewed Five (5) residents all who stated that their needs are being met and that facility staff ensure that proper house keeping is met daily. Based on the interviews and observations conducted LPA determined allegation to be Unsubstantiated.

Allegation four: Facility administrator is not qualified.
Licensing Program Analyst (LPA) Paola Guerrero conducted a record review and observed administrator certification, 40hr certification class, and further documentation to determine that the assigned Administrator possess the required requirements and documentation to be qualified. LPA Paola Guerrero interviewed Five (5) staff members who all stated that facility is under new ownership and the new Administrator ensures that the facility needs are being met. Staff also stated that the new administrator has good attitude and cares for the residents. Based on the interviews and observations conducted LPA determined allegation to be Unsubstantiated.

Allegation five: Facility is malodorous.
Licensing Program Analyst (LPA) Paola Guerrero conducted a tour of the facility and was accompanied by Facility Administrator Rebecca Parra. While on tour LPA observed facility to be in good condition. LPA inspected room 102, 107, 108, 104, and 110 to be well furnished and clear of obstruction and odor free bathrooms were also clean and well sanitized. LPA tour dining area and activity room area all to be in good and clean condition and free of odor. LPA interviewed five (5) staff members who stated that house keeping is done daily and rooms are cleaned daily. LPA interviewed five (5) residents who stated facility ensures that their hygiene is maintained daily to prevent odors. Residents also stated that at times some residents refuse diaper change and at times odor can be present. Residents also stated that caregivers ensure that residents remain clean to avoid odors. Based on the interviews and observations conducted LPA determined allegation to be Unsubstantiated.
