Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced visit to the facility to initiate a complaint investigation on the above allegations. LPA met with Administrator Michael Garcia and informed the purpose of the visit.

Regarding the allegation, Staff did not transport resident to appointments, interviews with seven (7) residents reveal residents are provided transportation to appointments. Interviews with outside parties, Administrator and three (3) staff reveal that the Resident #1 (R1) did have outside transportation arranged by the R1's POA for their 8/09 and 8/12 appointment. On 8/12/24, R1 had an appointment; however; it was not clearly stated to the Administrator or staff that outside party transportation was not being provided for this appointment.

Regarding the allegation, staff stole resident's phone, interviews with outside parties, seven (7) residents, the Administrator, and three (3) staff reveals not enough evidence to corroborate the allegation.

Based on LPA observations and interviews, the above allegations are Unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced visit the facility to initiate a complaint investigation on the above allegation. LPA met with Administrator Michael Garcia and informed the purpose of the visit.
Regarding the allegation, staff did not ensure the facility has adequate lighting in resident's bedroom, it was alleged that a resident had a broken hallway light and did not provide sufficient closet lighting. One (1) resident interview reveals that the hallway light in their bedroom was not working and the the facility took a few days to fix it. LPA observed a non-operating hallway light in bedroom #102 which provided insufficient closet lighting . Maintenance repaired the light during LPA's visit.

Based on LPA observations and interviews, the above allegation is Substantantiated. A finding that the complaint is Substantied means that the allegation(s) is valid because the preponderance of the evidence standard has been met.
An exit interview was conducted where reports (LIC9099&LIC9099-D) were discussed and provided with appeal rights to the administrator at the conclusion of the visit.
