Licensing Program Analyst (LPA) Paola Guerrero arrived at the facility to deliver investigative findings. LPA met with Facility Administrator Dana Jr Walker and explained the purpose of the visit regarding the allegations listed above.

First allegation: Facility staff hit a client in care. Regarding the allegation “Facility staff hit a client in care” LPA conducted interviews with Clients #2-4 regarding the alleged allegation and Clients #2-4 denied being hit by staff in addition, Clients #2-4 also denied witnessing staff hit client[s] in care. C#2-4 also informed that they feel safe and have no issues to report. LPA conducted interviews with Staff #1-7 LPA went over the alleged allegation with Staff and all denied hitting client[s] in care. In addition, Staff #1-7 also denied witnessing staff hit or mistreat clients in care.

Second allegation: Facility staff bully a client in care. Regarding the allegation “Facility staff bully a client in care” LPA conducted interviews with Clients #2-4 regarding the alleged allegation and Clients #2-4 denied being bullied or mistreated by staff furthermore, Clients #2-4 also denied witnessing staff bully

LPA conducted interviews with Staff #1-7 LPA went over the alleged allegation with Staff, and all staff denied the allegation of bullying of the client[s]. In addition, Staff #1-7 also denied witnessing staff mistreat or bully clients in care. Based on corroborating evidence the department has determined that the above allegations are Unsubstantiated, meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

An exit interview was conducted where this report (LIC 9099) was discussed, and a copy was provided to

Assistant Administrator Angela Calhoun at the end of the visit.
