13 | On 4/16/2026 at 8:50 AM, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility to investigate and deliver the findings of the above allegation. LPA met with executive director Monique Del Junco to explain the purpose of the visit. The investigation consisted of file review, interviews with facility staff and resident as well as facility observation.
Allegation: Staff are withholding resident's medication – Based on record review and interview with resident# 1(R1) and staff. It was revealed that facility doesn’t have any medication to administer to R1 because the pharmacy has not refiled R1’s medication for nonpayment from the healthcare provider. The staff offered to send R1 to the hospital to get temporary supply of medication until the resident can sort out the medical insurance issue but R1 refused because R1 thought R1 has to pay money to the hospital. LPA was unable to corroborate the allegation.
Based on the evidence, the allegation mentioned above is UNSUBSTANTIATED. A finding that the complaint is UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated at this time.
An exit interview was conducted where this report, LIC9099 was discussed and provided to executive director Monique Del Junco.
