| Allegation 3: Resident was found covered in feces - The POA reported finding R1 in feces on multiple occasions. All interviewed residents denied being left in feces or witnessing such issues. Staff denied leaving residents soiled and described routine checks every 30–60 minutes. This allegation is unsubstantiated.
Allegation 4: Staff did not notify responsible party regarding resident's falls.- Residents interviewed denied falling or seeing residents who fell. Staff reported all falls involving R1 were communicated to the POA and responsible parties. Copy of incident report was submitted to CCLD. The POA did not provide specific unreported falls. This allegation is unsubstantiated.
Allegation 5: Staff do not maintain sufficient food supply for residents in care. - Residents reported receiving adequate meals and snacks. One resident on a renal diet noted limited choices but stated staff were addressing this. Staff reported ordering food twice weekly. Facility provided invoices of food ordered. LPA observed sufficient food supply for residents in care. This allegation is unsubstantiated.
Allegation 6: Staff are not trained to conduct CPR. - Staff interviews indicate CPR/First Aid training is completed and maintained. The facility director stated files are reviewed monthly and provided proof of CPR certificate. No evidence indicated staff lacked CPR certification. This allegation is unsubstantiated.
Allegation 7: Staff are not providing showers due to insufficient staff coverage. - Residents reported receiving showers according to their schedules or showering independently. POA concerns were related to preferred shower times, not lack of showers. Staff reported adequate coverage and provided resident’s shower schedule. This allegation is unsubstantiated.
Allegation 8: Staff did not immediately contact 911 during an imminent threat to resident's health. - The POA stated 911 was not called until after POA instructed staff. Staff stated 911 was called immediately. No documentation or witnesses confirmed the timing. This allegation is unsubstantiated.
Based on interviews and record review, the allegations mentioned above are UNSUBSTANTIATED. A finding that the complaint is UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated at this time.
An exit interview was conducted where this report, LIC9099 and LIC9099C were discussed and provided to administrator Delcie Mucha.
