13 | On March 15, 2023, Licensing Program Analyst (LPA) Kuliema Calloway made an unannounced visit to LA Petite Academy. The purpose of the visit was to conduct a complaint follow-up inspection visit to deliver findings for the above allegation(s). LPA met with S1 who granted access.
During the investigation. LPA conducted interviews with children, parents, and staff. Based on the statements obtained during the interviews conducted there was documentation stating C1 caused an self-inflected injury by pulling a scab on a scar causing it to bleed. Staff communicated with P1 for duration of child’s enrollment regarding behavior. There was no evidence provided that C1 was bitten by another child. Staff disenrolled C1 for the safety of the children in care. Based on the evidence , the three allegation(s) on Neglect/Lack of Supervision and Personal Rights are Unsubstantiated meaning, the allegation(s) may have happened or are valid, but there is not a preponderance of evidence to prove that the alleged violation occurred. There are no deficiencies cited during this visit.
Exit Interview was conducted and A copy of this report, Notice of Site Inspection, and Appeal Rights were discussed and left with S1 at the facility.
