25 | On September 7, 2023, at 12:08 PM., Licensing Program Analyst (LPA) Kuliema Calloway conducted an unannounced Case Management Incident inspection and met with facility director who granted access. LPA observed seventy one (71) day care children in active play and eleven (11) staff present during the inspection.
The purpose of the inspection was to follow up on a self reported unusual incident report (UIR) that occurred on August 30, 2023. The Department was notified of the UIR on August 31, 2023. The UIR stated, Child 1 (C1)'s grandmother phoned the child care center to say C1 tested positive for lead and notified the Health Department.
On May 17, 2022, there was a lead in drinking water assessment provided by A-Tech Consulting Inc., where on April 23, 2022, a water sample was taken on five water fountains inside the facility. Per the water assessment report, the fountain that tested high for lead was identified inside an infant/toddler classroom. Per the Director, the fountain that tested high for lead inside the infant/toddler classroom was immediately replaced with a faucet that allows hand washing only. LPA observed the fountain faucet that was identified. Per the Director, the water fountain C1 possibly drank from was inside the two year old classroom and that fountain tested normal for lead exposure. LPA observed the drinking fountain inside the classroom and compared it to the facility sketch on the water assessment and it does not identify as the same faucet that tested high for lead exposure. |
