On August 9, 2023 at 09:05 a.m. Licensing Program Analyst (LPA) Kendal Zirbes met with Facility Representative Kecia Loving. The purpose of the inspection was to conduct a required one year inspection at the licensed facility. LPA disclosed the purpose of the inspection to the facility staff member. Present during today’s inspection were 68 preschool age children, seven teachers, and additional staff members.
The preschool area consists of five classrooms, three children's restrooms and two outdoor play spaces. The Center also has a licensed infant center component (364809116) and school age component (364801076). All three components share the front office space, administrative offices, kitchen, staff break room, and staff restrooms. The center operates 06:30 a. m. - 06:30 p. m. Monday to Friday.
Indoor/Children's Area: LPA observed all outdoor and indoor classrooms, passageways, stair ways, incline, ramps, open porches and other areas of potential hazard are kept free of obstruction; floors of all rooms have a surface that is safe and clean, cleaning compounds are stored in each classroom. The cleaning products in each classroom were stored in cabinets equipped with key locks. In the Pre K classroom, LPA observed two spray bottles containing cleaning solution. One spray bottle consisted of a soap and water solution and the other was a bleach and water solution. The spray bottles were observed on a shelf behind a unlocked closed cabinet door. The shelf was approximately three feet high. Children were in this classroom and engage in an art activity with the teacher. Furthermore, in an unoccupied preschool classroom, LPA observed two spray bottles with cleaning solution on a 3"10' shelf, in addition four additional spray bottles with cleaning solution were observed in an open cabinet on a shelf that was between 2-3 feet high. LPA observed children use this classroom as a passageway. LPA observed Bleach and cleaning solutions in a cabinet under the kitchen sink. The cabinet was not equipped with a locking mechanism. The kitchen door was open, and no barrier prevented the children from entering the kitchen. Two staff members were present in the kitchen when LPA observed the cleaning products under the sink. When LPA observed the cleaning products the children were under a teacher’s supervision. Based on LPA's observation the cleaning products were potentially accessible to the children in care. A Type B citation was issued as the cleaning products were a potential risk to the children in care. Report continued on page two

Zero poisons were stored within the preschool center. The furniture was good condition. The Center was free of flies, other insects, or rodents. Each classroom was equipped with tables and chairs to meet children’s needs. All play equipment and materials used by children are age-appropriate, children have individual cubbies labelled with their name for their clothing, personal belongings and or bedding. Trash cans for solid waste have tight fitting lids. All materials and surfaces are toxic free. No fireplaces were present in the preschool classrooms. Carbon monoxide detectors, smoke detector and fire extinguishers were located throughout the Center. The Center had working telephone service on the premises. There is an isolation area for children who become ill while in care. The facility maintains a comfortable temperature at all times. Sign in/out sheets were available and are completed each day. Smoking prohibited on the premises. Staff complete daily inspections for illness. LPA did not observe prohibited child care items at this time. Firearms/weapons are not allowed or stored on premises. There are no bodies of water on the premises. The Center has the menu posted. Children have access to water while in the classrooms via drinking fountains.
During LPA's walk through LPA observed the sink and faucet in the 2's classroom had rust and hard water deposits around the knobs. Based on information obtained from the facility representative, the Center was being remodeled over the weekends. Facility representative stated, the sinks, faucets, counter tops, toilets and bathrooms are in the process of being repainted and new fixtures are on back order. Per facility representative the remodel should be completed by the end of August 2023. LPA provided a technical advisory notice to the Center as the Center has a plan for replacing the items that are showing signs of wear. LPA advised that the Center had to be maintained in a clean, safe and sanitary environment. LPA advised the Center that additional staff should be on site to completed housekeeping and maintenance functions.
Napping: Cots are utilized by the children.
Restrooms: There are four children's restroom in the Center. The Twos classroom was equipped with a changing table. Additional restrooms available for the staff. In the Preschool bathroom which is shared with school age program, LPA observed the toilet paper rolls were sitting on top of the toilet paper dispensers. In the Pre K 2 classroom, LPA observed the paper towel rolls were placed on top of the dispensers. LPA advised the Center of their responsibility to ensure a safe and sanitary environment. An advisory notice was issued.

Kitchen: The facility has a self contained kitchen. The kitchen has a door which prevents the children from access the kitchen area. LPA observed the kitchen door was open during this inspection. LPA advised the facility to ensure the door remains closed and lock so the knives and chemical are not accessible to the children in care. Kitchen has a sink, refrigerator, a stove, and storage cabinets. The Center provides breakfast, lunch and one snack.
Outdoor: The preschool has two enclosed outdoor play spaces. One of the spaces is used specifically for the twos classroom. LPA observed plastic bags and trash debris in the corner of the twos outdoor play yard. The outdoor trash can was full and the lid could not be closed all the way. Ten children and one teacher were playing in the outdoor area when LPA made these observations. LPA advised the Center to ensure all debris is removed from the play space prior to the children playing in the area and ensure the trash is emptied each day if the lid can not close. A technical advisory notice was issued.
A second outdoor play space is utilized for the older preschool age children. The space has a large climber placed on wood chips, a grassy area for large motor movement, shady areas and benches. The climbing structure was securely anchored to the ground. LPA advised the Center to ensure the depth of wood chips under the climbing structure can absorb falls. A technical assistance was provided. Zero bodies of water were observed. The outdoor play space is completely fenced and separate from the playground utilized by the infant and school age components. Drinking water is readily available to the children when outside via drinking fountain and a water jug and cups.

During the inspection teacher child ratios were observed, and found to met the requirements. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate.

Staff/Personnel Records
LPA reviewed five child records and six staff files during this inspection. Child records were complete and contained all required information. Five of six staff files reviewed were did not contain documentation that the staff completed the two year renewal of the mandated reporter training. Two of six staff files reviewed were missing documentation that the staff had received or declined the influenza vaccination between August 1 and December 1 of each year. LPA and Facility representative reviewed the LIC 311A Records To Be Maintained At The Facility.
The facility has met the required Lead testing requirements. Per conversation with the facility, the Administrator/Director on record with the Department no longer works at the Center. LPA advised the Center to submit the required documents to the Department so the name of the Director/Administrator can be updated. Per facility representative the Center previously reported the change in Administrator to the Department.

Health Related Services
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Postings:
The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393), car seat law (PUB 269), emergency disaster plan. The Disaster and Mass Casualty Plan was posted. Per LPAs review the plan was completed in 2014 and the staff members listed on the plan do not currently work at the facility. LPA advised the Center to update the Disaster and Mass Casualty Plan. A technical violation was issued.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. Facility representative was advised of the requirement to report Unusual Incidents. Facility representative informed to utilize the Unusual Incident Report/Injury Report LIC624 when submitting the report to the department (email address: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. The On-Duty Worker is available for questions at (661) 202-3318 Monday through Friday 8am-5pm.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on LPA observations and record review three type B citations, four technical violations and one technical assistance was issued.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the facility representative Kecia Loving.
