On August 22, 2023 at 09:55 a.m., Licensing Program Analyst (LPA) Kendal Zirbes conducted an unannounced Plan of Correction (POC) inspection and met with facility representative Kecia Loving. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Facility Representative. 58 preschool age children were present in the facility during this inspection.

During this inspection, LPA and facility representative Kecia Loving completed a tour of the facility at approximately 09:59 a.m. LPA observed the following:
1. In the Pre K classroom, a cleaning caddy with four spray bottles containing cleaning solutions, were observed in an unlocked closed cabinet in the classroom. The shelf was approximately 2-3 feet high. 12 children and one teacher were observed in the Pre K classroom. In the preschool classroom, LPA observed a bottle of toilet cleaner on the floor of the school age bathroom. The door of the school age bathroom was open and a barrier was not present preventing the preschool age children from accessing the cleaning product. LPA observed 11 preschool age children and one teacher in the preschool classroom. In the transitional kindergarten classroom, LPA observed a cleaning caddy with four spray bottles of cleaning solution in a unlocked cabinet with the doors closed. Inside the cabinet the cleaning solutions were on a shelf that was approximately 3 feet high. The cabinet doors were not equipped locking mechanism, therefore the cleaning products were accessible to the children in care. LPA observed 12 children and one teacher in this classroom. At 11:00 a.m., LPA observed the door leading to the kitchen was open. Cleaning products are stored in a cabinet under the kitchen sink that was not equipped with a child safety lock. Cleaning products in the kitchen were accessible to the children in care in the Pre K classroom.
During this inspection, LPA received a copy of the sign in sheet for the training completed 8/12/23.
Based on LPAs observation, cleaning products were accessible to the children in care. On 8/9/2023 the facility was cited for regulation 101238(g), due to cleaning products being accessible to the children in care. The plan of correction was due 8/15/2023. Due to the facility failing to correct the deficiency by 8/15/23, a civil penalty was issued. Refer to LIC421FC.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative Kecia Loving.
