32 | Staff further stated the child was not involved in any additional incidents that would have resulted in facial or ear injuries. The facility was unable to provide a definitive explanation for how or when the injuries occurred or confirm whether the injuries occurred while in care. It was noted a similar incident was recently addressed during a previous inspection on July 28, 2026 which resulted in, no deficiencies cited.
The complaint also alleged staff are not meeting the child’s toileting needs, specifically reporting the child has been picked up multiple times with feces in their underwear. Pertinent Parties disclosed staff supervise restroom use by standing at the doorway to ensure safety and to determine whether children require assistance. Staff reported children are encouraged to wipe themselves during potty training; however, if children exhibit signs of needing help, staff provide assistance, and accidents are followed by a thorough cleanup. The Director stated potty-training children are taken to the restroom every 30 minutes to an hour or as needed in between, as well as before transitions, outdoor play, and nap time. The facility uses the KinderCare app to document toileting, although staff reported documentation is not always consistent due to the frequency of restroom use. In this case, information was obtained that the child had two accidents but not recorded or reported. A review of the child's file did not reveal or indicate the child requires special toileting accommodations.
This agency has investigated the complaint. Based on the conflict of information, interviews conducted, and documentation reviewed, the allegations are UNSUBSTANTIATED. A finding that the allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the allegation occurred.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
