32 | On 07/23/2026, LPA conducted an unannounced visit to the facility to initiate this complaint investigation. During this visit, LPA conducted interviews with pertinent facility staff.
Staff admitted that on this specific date, 07/17/2026, staff witnessed an incident regarding a lapse of visual supervision of a child in care. Staff stated they had entered the outdoor play area and personally observed a child in care to be alone and located on the opposite end of the playground. It was stated the child had wandered away from a group of children within the outdoor play area. Staff stated the staff member, who was present on the playground at this time, was unaware the child had wandered off, resulting in a lack of supervision for an undisclosed period. Staff stated they retrieved the child and reunited them with the other group of children.
Regarding inappropriate comments made by staff pertaining to a child’s personal/physical characteristics, staff admitted, on 07/17/2026, they made negative comment about a child, outside the child’s presence, to another individual.
Based on LPA's observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above listed allegations: Staff did not provide adequate supervision to children in care and Staff did not provide dignity to children, are found to be SUBSTANTIATED.

A NOTICE OF SITE VISIT WAS GIVEN. DIRECTOR WAS INSTRUCTED TO POST IT IN A PROMINENT LOCATION AT THE FACILITY. THE DIRECTOR UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS.
An exit interview was conducted, A copy of this report and appeal rights were given to Facility Director, Christa Martinez on 09/02/2026.
