This is an amended report to make updates on a report originally given to the facility on 3/13/25.
Licensing Program Analyst (LPA) Zirbes and Cha met with Facility Representative Liliana Mendez for the purpose of conducting an unannounced required annual/random inspection for the preschool. LPA toured and inspected the facility in accordance with the facility sketch. There were seven preschool age children, one teacher, and two substitute aides on the premises when LPA arrived. The preschool program is operated out of one modular classroom (PS 1). The classroom is located on the campus of Arrowhead Elementary school. The programs hours of operation are Monday thru Friday 8:00-4:00pm . The preschool program has two sessions each day, AM session 8:30 AM -11:30AM and PM session 12:30 P.M -3:30 P.M.
Indoor/Children's Area:
Child care center was observed to be clean, safe, sanitary and in good repair; all outdoor and indoor passageways, stair ways, incline, ramps, open porches and other areas of potential hazard are kept free of obstruction; floors of all rooms have a surface that is safe and clean. Cleaning compounds were stored in key locked cabinets in the classroom. LPA observed the furniture/equipment was in good condition. Classrooms were free of flies, other insects and rodents. The classrooms had tables and chairs to meet the children's needs. LPA observed the Center has a variety of art supplies, dramatic play area, computers area, reading center, quite area, and a variety of additional educational materials. Children's belongings are kept in cubbies labeled with the children's names. Trash cans for solid waste have tight fitting lids. Drinking water is readily available indoors via two drinking fountains in the classroom. All materials and surfaces are toxic free, window screens are in good repair and free of debris. There was a carbon monoxide detector, and fire alarm in the classroom. There was working telephone service within the classroom. There is an isolation area for children who become ill. The facility maintains a comfortable temperature at all times. LPA observed the centers electronic sign in/out sheets, which are completed daily. Smoking is prohibited on the premises. Teachers conduct daily inspection for illnesses. LPA did not observe prohibited childcare items during this inspection. Firearms/weapons are not allowed or stored on premises. Facility representative stated no bodies of water on the premise. Report continued on page two

Restrooms: The Classroom has a total of two children restrooms and one staff restroom. Each of the children's restrooms was equipped with one toilet and one sink. Restroom #1 was observed to be in safe and sanitary operating condition. Paper towels and soap were available to the children. In Restroom #2 LPA observed a water leak from the ceiling due to heavy rain weather. Facility representative was in process of reporting the issue for repair, upon LPA arrival. Restroom #2 was made inaccessible to children in care by closing the door and utilizing a bin to prevent flooding.
Meals: The Center provides one meal during each of the sessions. The meals are prepared in the school cafeteria. Menus were posted in the classroom.
Napping: This is a half day program therefore, the children do not nap while in care. A quite area was available in the classroom for those who need a quite activity.
Outdoor
The preschool program utilizes Arrowhead Elementary School kindergarten outdoor play area. LPA inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness . The area has one large Playworld play structure for children between two through five years old. The large play structure has six slides. The cushioning material under the play structure was rubber matting. The large play structure was securely anchored to the ground. The area was observed to be free of debris at the time of this inspection. Drinking fountains are located within the play yard for the children.
Personnel Records
LPA reviewed six child files which contained all required information. LPA reviewed three personnel files during this inspection. Per LPA review, the staff Mandated Reporter training was completed through an approved outside vendor.
Per LPA record review, Staff 1 (S1), Staff 2 (S2) files were missing influenza vaccination declinations for August 1, 2024 through December 1, 2024. LPA discussed with facility representative about the requirement to have influenza vaccination declination documentation in all staff files. A Technical Violation was issued. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

As a result of today's inspection, one Technical Violation was issued.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Liliana Mendez. Appeal Rights were also provided.
