Child /Personnel Records
LPA reviewed five child record and two personnel files during this inspection. Based on LPA review, all child files contained the required information. Staff 3 (S3) personnel record was not provided to LPA during this inspection. Per facility representative S3 has worked at the Center for at least two days, LPA observed S3 interacting with the preschool children. A type B citation was issued for regulation 101217(d) Personnel Records, refer to LIC 809D. A staff person was on site with current CPR/First Aid.
Facility Records
LPA and Facility representative reviewed the LIC 311A Records To Be Maintained At The Facility. LPA observed a current roster, disaster drill completed in October 2023. The required lead testing was competed. Per LPA review the facility roster was missing the name and telephone number of the child's physician and per conversation with the facility representative written documentation of the disaster drills could not be located. Advisory notices were issued.
Posting Requirements
The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393), menu, car seat law (PUB 269), emergency disaster plan, earthquake preparedness checklist.
Health Related Services
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The Center shall permit the Department to inspect the child care center and to privately interview children or staff, to determine compliance with or to prevent violations of child care laws or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.
