Licensing Program Analysts (LPA) Zirbes and Cha met with the facility representative Carina Diaz for the purpose of conducting an unannounced Annual/Random inspection. LPA toured and inspected the facility in accordance with the facility sketch. There were 12 preschool age children, one teacher and one aide on the premises when LPAs arrived. Based on LPAs observation the facility was meeting the capacity/ratio requirements during this inspection. The preschool program is operated out of a bungalow (PS 2) located on the Bradley Elementary School campus. The facility operates two session each day, an AM session, 8:30am - 11:30 am and the PM session operates 12:30 PM - 3:30 PM.
Indoor/Children's Area: Child care classroom was observed to be clean, safe, sand and in good repair; all outdoor and indoor passageways, stair ways, incline ramps, open porches and other areas of potential hazard are kept free of obstruction. Cleaning compounds were stored in a cabinet equipped with a child safety locks. Therefore the cleaning products were inaccessible to the children in care. Zero poisons are stored in the classroom. LPA observed the furniture/equipment is in good condition. Classroom was free of flies, other insects, rodents. The classroom had tables and chairs to meet children’s needs. LPA observed the classroom had designated areas for dramatic play, art, writing, manipulates, library, science and a computer area. In addition, there was a carpet area for story/instructional time. Children's belongings are kept in cubbies labeled with the children's names. Trash cans for solid waste have tight fitting lids. Drinking water is readily available indoors via a drinking fountains, water cooler and cups. All materials and surfaces are toxic free, window screens were in good repair and free of debris. There was a working carbon monoxide detector in the classroom. When LPAs reviewed the contents of the First Aid kit, LPAs observed aspirin and non-aspirin along, eye wash solution and antiseptic solution in the first Aid kit. These items were labeled keep out of reach of children. The first aid kit was located on shelf that was 33.5 inches tall. The LPAs did not observe the children walk unsupervised within six feet of the location of the first aid kit. During the inspection, LPAs observed the children were under the supervision of a staff member at all times. Based on LPA observation, the non prescription medications were a potential risk to the children in care, refer to LIC 809D. Report continued on page two

During this inspection the drinking fountain was covered with a red cap, therefore the drinking fountain was made inaccessible. In the future the facility representative will conduct a morning check prior to children arriving and will notify the District if the drinking fountain is accessible by 08:30am. There was working telephone service within the classroom. There is an isolation area for children who become ill located within the classroom. The facility maintains a comfortable temperature at all times. Smoking is prohibited on the premises. LPA did not observe prohibited childcare items during this inspection. Firearms/weapons are not allowed or stored on premises. LPA did not observe bodies of water on the premises. Meals are provided through the schools food program.
Napping
The preschool program only offers half day programs. Therefore, the children do not nap at the facility. The classroom has a designated areas for relaxation and quite play.
Restrooms: The classroom has two children's restrooms and one staff restroom. Each of the restrooms had one toilet and one sink. At approximately 10:05am, LPA's inspected the children restrooms and observed some dirt spots on the two walls adjacent to the toilets, faucets and sinks (2), and toilets (2). The white toilets were discolored. Based on LPA observations, the children's restrooms showed wear and tear. Per facility representative, restrooms are cleaned daily. Facility representative notified LPAs that work orders for new toilets were made and a deep cleaning of the sinks. Administrator expects work orders to be completed during spring break of March 2025. LPA advised center to notify department when work orders are completed. A Technical Violation was issued based on LPA observation and information acquired from interviews.
Outdoor Activity Space: LPAs inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness. The area has one large play structure, and a large grass area for gross motor activities, and benches/tables with umbrellas for shade. Rubber surface matting was the cushioning material under the play structure and the play structure was securely anchored to the ground. The area was observed to be free of debris at the time of this inspection. LPA Zirbes toured the outdoor activity area at approximately 09:00am. LPA observed two drinking fountains, and one water bottle refill station in the outdoor activity area. According to information received from the facility representative staff have been advised that the outdoor water station should not be utilized by the children. Facility representative stated, the children drink from water bottles brought from home or from cups and water cooler in the classroom when the children engage in outdoor activities. LPA reviewed documentation regarding the Centers water testing completed in 2023. Per testing results, one outdoor drinking fountain (Outlet E) had an ALE of 5.7000, which is out of compliance with the allowable ALE of 5.5 ppb.

Outdoor activity space continued: On April 7, 2023, the Center agreed to keep outlet E inaccessible to the children via a red cap covering the fountain. On December 3, 2023, LPA verbally advised the Center to ensure drinking fountain (E) is inaccessible at all times. Outlet E a contaminated fixture, was accessible to the children in care on this day, this is a immediate risk to the children in care , therefore a Type A citation was issued, refer to LIC 809D. LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.
File Review: LPA reviewed ten child records and two personnel files during this inspection. Based on LPA review, child and staff files contained the required information.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

As a result of today's inspection, one type A citation, one type B citation and one advisory notice was issued.
LPA Zirbes informed facility representative Carina Diaz that this report dated February 26, 2025, documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Zirbes informed the Licensee to provide a copy of this licensing report dated February 26, 2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Carina Diaz. Appeal Rights were also provided.
