Licensing Program Analyst (LPA) Ibitoye met with Lead Teacher (LT), Adriana Franco today for the purpose of conducting an unannounced Annual/Random inspection. This Pre-school program is held on the grounds of Newmark Elementary school in bungalow PS9. Staff are fingerprinted through the San Bernardino School District. Present today were 18 children 3 Staff. Hours of Operation: MONDAY THROUGH FRIDAY; 8:30 A.M. - 11:30 A.M. AND 12:30 P.M TO 3:30 P.M.

**Telephone service was verified. Heating, lighting, and ventilation are adequate. Children's belongings are kept in cubbies as you enter the classroom. LPA observed age appropriate toys and materials. The child care center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees and visitors. The flooring in the classroom has a surface that is safe and clean. Drinking water is available inside the classroom in the form of water bottles. water cooler and disposable cups.

**Center serves breakfast and lunch (AM/PM classes) which are delivered from the school cafeteria. Menus are posted.

**There are 2 separate bathrooms designated for children located within the classroom, each bathroom has 1 toilet and 1 sink. There is a 3rd bathroom designated for Staff. LPA inspected and observed bathrooms to be sanitary, Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available.

Center/Parents utilize an App called MSign to sign children in and out daily via tablet. There is a fully stocked first aid kit available and fully charged fire extinguisher. Fire Drill/Earthquake log is posted, last drill conducted 04/30/2024, CPR/First Aid (02/07/2025) and Mandated report training(06/07/2025.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. *Trash cans/storage containers for solid waste had tight-fitting covers that are kept on, and in good repair

** Outdoor play equipment was inspected for health, safety, cushioning material, good repair and age appropriateness. Center utilizes rubber matting under climbing structures for cushioning material. Large play equipment and structures are securely anchored. There is a shaded patio area with benches; drinking fountains available on the play yard. on the campus.

**Children are inspected for illnesses as they arrive. A review of medication policy indicated that only prescription medication is administered and only with parent's written permission. The director/lead teacher administers the medication and document the dosages, date and time onto a log. Incidental Medical Services were discussed. Medication is properly labeled and stored in its original container. There is a separate area for isolation and care of ill children as needed.

A sampling of Children's records were reviewed, The main office staff will forward staff files for review via email. LPA Review 1 Sub-Teacher file present. See LIC 9102.

Teacher- child ratios were observed and staff names were recorded. Care and supervision were evaluated to determine if the basic needs of children were met and appropriate.

The facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

The facility representative was informed of the MyChildCarePlan.org website, a
consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

All staff are required to take 3-hour Child Care Provider Mandated Reporter training (AB1207) every 2 years. Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors, and employees to complete training as specified in their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from the date of employment to complete training as required. The training may be conducted at the following website www.mandatedreporterca.com. The director is aware self and all staff are mandated child abuse reporters and have the responsibility of reporting any suspected child abuse to the Child Abuse Hotline at (800) 540-4000.

The Lead Teacher advised of the requirement to report Unusual Incidents. The lead Teacher was informed to utilize the Unusual Incident Report/Injury Report LIC624B when submitting the report to the department (email address on the website: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of the daycare center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. An On Duty Worker is available for questions at (661) 202-3318 Monday through Friday 8 am-5 pm.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The director shall permit the Department to inspect the family child care home and to privately interview children or staff, to determine compliance with or to prevent violations of child care center or regulations, also enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.
