On November 30, 2022 at 1:25pm Licensing Program Analyst (LPA) Zirbes met with the Site Supervisor Annabell Lopez Magana. The purpose of the inspection was to conduct an unannounced required one year inspection. During the inspection, LPA toured and inspected the Infant/toddler program in accordance with the facility sketch. LPA observed a total of 17 children (four infants, and 13 toddlers) with two teachers and four aides providing supervision. The infant/toddler area consists of one classroom with separate spaces for the infants and toddler, two infant/toddler bathrooms, one staff bathroom and an outdoor space. The center operates 07:00 AM -05:30 PM Monday -Friday.

Indoor/Children's Area: Infant/toddler center is equipped with appropriate furniture and equipment, changing table and feeding chairs. Furniture is maintained in good repair and safe condition (age appropriate). Toys are safe and LPA did not observe sharp points or edges, or small parts that could be swallowed. Per Site Supervisor equipment, toys and furniture are sanitized daily. There is one hand washing sink within arms length of the changing table. No infant is left without supervision (visual), nor left unattended. The facility is in compliance with the staff/infant ratio and is not exceeding the approved capacity for the facility.

Furniture and equipment were inspected for age appropriateness and good repair. LPA observed the three child stroller was stained and worn. LPA advised the center that the stroller was showing signs of disrepair and the Center should look into replacing the stroller. Per Site Supervisor, a new stroller has already been purchased and just needs to be assembled. LPA observed the infant changing table was in good repair. Drinking water is available inside the classroom (sippy cups, bottles). Used diapers are put in a closed diaper container within an arms reach of the diaper changing table. Each infant/toddler has a container labeled with their name for the storage of personal items. There were evacuation crib in the sleeping room.

LPA observed Tide, carpet cleaner, floor cleaner, and dish soap in the closed cabinet under the kitchen sink. The lock to the cabinet was not working at the time of this inspection therefore the cleaning products were accessible to the children in care. LPA observed the infants and toddlers in care were never closer than eight feet from the cabinet where the chemicals were stored. The Center had additional staff present at the time of this inspection providing care and supervision to the children. Based on LPAs observation the cleaning solutions posed a potential risk to the children in care.
The Center does not have a fireplace. The building has central heating and air conditioning units. The room temperature was comfortable at the time of this inspection. LPA observed the carbon monoxide, Fire Extinguisher and smoke detector in working condition. There is a working telephone on the premises. There is an isolation room for ill children. No smoking is allowed on the premises. LPA did not observe baby walkers or other prohibited play equipment on the premises.

Children/Infant are inspected for illnesses (wellness policy) as they arrive. A review of medication policy indicated that prescription and non prescription medication are administered only with parent's written permission and a doctor's note. Staff document when the medication is administered. All medications are stored in a locked container. At the time of this inspection, no medications were administered

Napping: LPA observed mats, cots and cribs. Per Administrator, children over one years of age are transitioned to a mat or cot on the floor. LPA observed the crib mattresses were made of a cleanable vinyl material and were in good repair.

Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate.

Outdoor: Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. The outdoor area, has grass, dirt, and cement for active play. At the time of this inspection, the Center had climbing equipment which was placed on cushioned material. There were zero bodies of water are on the premises. Outside area is completely fenced. At the time of this inspection. the outdoor space was is maintained in a safe condition and is free of hazards.

Posting Requirements: Center has required posting posted (license, PUB 393, LIC610, car seat law information).

Based on LPA observations one type B citation was issued today for regulation 101238(g). Refer to LIC 809D.

LPA was unable to complete the entire inspection today therefore LPA will return to the Center on a future date to review child/staff file, and evaluate additional components of the program.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Site Supervisor, Annabell Lopez Magana

Based on observation, the licensee did not comply with the section cited above when LPA observed Tide, carpet cleaner, floor cleaner, and dish soap in the closed cabinet under the kitchen sink. The lock on the cabinet was not working at the time of this inspection and the chemicals were accessible to the children in care, this posed a potential health, safety or personal rights risk to persons in care.

The cleaning products were removed at the time of this inspection to a locked storage shed. A work order have been submitted to have the lock fixed on the cabinet. The chemicals will remain in the locked shed until the lock on the cabinet is fixed. A staff training regarding safety and communication will occur and Agenda will be submitted to the Department.
