LPA disclosed the purpose of the inspection and was granted entry into the facility by a facility representative. 10 toddlers, three infants and seven staff members were present in the facility during the inspection.
The purpose of the inspection was to follow up on a self-reported unusual incident report (UIR) which occurred on June 28, 2023 at the facility. According to the UIR, child 1 (C1), child 2 (C2), and child 3 (C3) were observed in the parking structure which is outside of the enclosed play yard. According to the UIR, the maintenance workers left the gate unlocked after they had been working in the play yard before the toddlers went outside to play. Per UIR, C1, C2, C3 were immediately returned to the play yard and the gates were secured.
During the inspection today, LPA interviewed four staff members and review C1, C2 and C3 files. Per facility representative the Center does not have video evidence of the incident.
LPA completed a safety inspection of the classroom and play yard at 9:25 a.m. Zero deficiencies were observed.
