On 8/11/2022 at 2:00 pm, Licensing Program Analyst (LPA) Patricia Berry conducted a case management incident investigation. LPA was granted access into the facility and met with director. LPA toured facility and took a census.

On 8/1/2022 the director self reported an incident alleging a teacher inappropriately handled a child. Director stated she conducted an in-house investigation and determined the allegation to be unfounded.. The police were called and interviewed staff and child. Director stated the police informed her the incident was unfounded.

Due to insufficient information at this time, LPA will return at a later date to deliver final report.

Exit interview with director, report, appeal rights and Notice of Site Visit provided to director.

LPA observed director post notice.
