Licensing Program Analysts (LPA), Aman Sharma conducted a required annual inspection. LPA was met with site director, Sabrina Katz. An inside out tour of the facility was given, and the following was observed and/or noted:
This center is licensed for an infant and school-age license as well, which was also inspected on this date.

· The site director is requested to update the following documents as needed, and submit to licensing within 30 days:
1. LIC 500 Personnel Report
2. LIC 610 Emergency & Disaster Plan
3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made)

Based on LPA's observation, one of the classrooms had the bottom border around the wall coming undone. The plastic piece has a sharp corner, which could be harmful to children in care. LPA observed toys near the plastic piece coming off of the wall. Additionally, the preschool playground had a big crack in the plastic climbing structure. This poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA's observation, there was dirt, debris and stains on the carpet inside the classrooms where children play. A spot the size of a baby car seat was observed on one of the rugs, while other rugs had multiple stains. These stains were brownish-black in color. There was a restroom in the preschool classroom that has a soiled bag of trash on the floor, immediately to the right of the door. There was also poop smeared on the toilet paper dispenser, which had no toilet paper in it. No toilet paper was observed in 2 out of 4 of the children's restrooms. The tables and floors do not look like they have been wiped down and are sticky to the touch. The floor tile had multiple stains and droppings of unknown substances, leaving the floor with dried up drops. The igloos used for uncontaminated drinking water was observed in classrooms, but empty and with grime and dust on the top of them. This poses an immediate health, safety or personal rights risk to persons in care.

Director has put in an emergency work order for the floors and carpet to be cleaned. Director also immediately had a staff clean out the restroom with poop on the toilet paper dispenser, remove the bag of trash and ensured there was toilet paper in each of the restrooms. Director will sumbmit pictures of the corrections to licneinsg no later than the POC due date. Director will implement a written plan, where she or the assistant director will check and sign off on cleaning duties for staff each day.

·Classrooms are adequately equipped with age and size appropriate furniture and equipment.
· There are no weapons present at the facility.
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Children bring their own water bottles to utilize in both, indoor and outdoor activity spaces.
· This center has not gotten their lead testing done, and children are still utilizing drinking faucets to re-fill their water bottles. The site director stated that teachers re-fill igloo jugs from an uncontaminated drinking fixture, which they bring into the classrooms during the afternoon. LPA noted the igloo containers in the classrooms, but none of them had any drinking water in them and were empty.
· Medications are currently being used at the facility-SEE LIC809D
· LPA observed hazards, which were stored where they were accessible to children. These include: Disinfectants, cleaning solutions and other items that are dangerous to the health and safety of children in care.
· Poisons and toxins are kept locked and inaccessible to children, per Title 22 Regulations.
· Floors were observed with spots, debris, and grime on them. Floors were also sticky, LPA noted while walking through the classrooms, her shoes were sticking to the floor and making a noise while walking. Carpets did not seem to have been vacuumed or cleaned for some time. Classrooms had large spots of spilled liquids on them, some with a brownish-black color -SEE LIC809D
· 3 out of 4 Restrooms were observed and in operating condition and 1 had a bag over the toilet, with the site director stating that there was a work order put in for that toilet. 2 out of 4 of the restrooms did not have any toilet paper in them, and one had poop smeared on the top of the toilet paper dispenser.
· Playgrounds are enclosed by appropriate fences.
· The outdoor activity area is in good condition and supplied with age/size appropriate equipment.
· The areas around or under high climbing equipment, swings, slides, and similar equipment were cushioned with material that absorbs a fall.
· Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin.
· Food is stored appropriately and protected from contamination.
· Menus were posted with date included and were placed in a visible location of children’s authorized representatives. Menus shall be kept on file for 30 days, and made available upon request.

·All storage containers for solid waste, including moveable bins have tight-fitting covers that were observed to be on and in good repair. One of the trash cans did not have an operating lid, which had exposed garbage of food, used bowls, plates, cups and napkins.
· Sign in/Sign out record was reviewed and meets regulation requirements.
· Some staff members had a current Pediatric CPR/First Aid on file.
· A review of children’s records were found to be complete during this inspection-SEE LIC809D.

· Disaster drills are to be conducted every six months – last drill was conducted on 10/10/23
· The Director was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov
· A review of staff records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
· The Director can submit transfer forms to associate or disassociate someone from their facility at: Associations_Disassociations862@dss.ca.gov
· A review of staff records indicates that staff that were present meet minimum qualifications for the position for which they were hired.
- This facility does currently provide Incidental Medical Services (IMS). LPA reviewed and discussed submission of IMS plan to the department. Facility understands children’s, personnel, and administrative records must be completed as needed, and the original packaging for medication must be kept on file. Type A citation(s) shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Additionally, LPA Aman Sharma, informed the director, Sabrina Katz, to provide a copy of this licensing report dated 10/23/2023, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
