25 | On the date and time listed above, Licensing Program Analyst (LPA) arrived at the facility on another matter. During the tour of the school age classroom, LPA noticed the cabinet on the far side of the room unlocked. LPA inspected this cabinet and saw chemicals stored in there for cleaning. At the time, there were no children in care and the staff in there was cleaning and organizing the room. Approximately 2 hours later, school age children had been picked up from their respective schools and they were sitting in the school age classroom and eating. LPA walked over to the same cabinet, on the far side of the room, and saw that it was still unlocked. There were two disinfecting sprays which both had "Medical Emergency" phone numbers to contact. Additionally, the following note was on them "CAUTION: Causes moderate eye irritation. Avoid contact with eyes or clothing. Wash thoroughly with soap and water after handling". The cabinet was about 4 feet high up, with the tallest child in the classroom being at least 5 feet tall. There was approximately 2 feet between the children sitting on a round table having their snack, from the cabinet.-SEE LIC809D
During another inspection conducted on another matter, LPA reviewed 4 children's files and 2 staff files. 3 out of 4 of the children's files were incomplete. Both of the staff files were incomplete-SEE LIC809D.
LPA Aman Lama informed the Director, Sabrina Katz to provide a copy of this licensing report dated January 9, 2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled, or newly enrolled by the next business day or the next day the child(ren) is(are) in care. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification and kept on file for 12 months from the date of this report.
Exit interview was conducted with site director, Sabrina Katz. A copy of this report, Notice of Site Visit, and Appeal Rights were provided. A notice of site visit must remain posted for 30 consecutive days.
