25 | On the date and time stated above, Licensing Program Analyst (LPA) Aman Sharma arrived at the facility on a case management inspection to follow up on an incident reported via telephone. The incident was called in on 07/21/23 by facility director, Sabrina Katz. Upon arrival, LPA was met with facility director to discuss the reported incident. LPA toured the facility, including the location where the incident took place, and took census.
It was disclosed that the incident occurred on 07/20/23 at approximately 9:00am. During transition from indoor to outdoor activity, a child was left indoors, in the toddler classroom, unsupervised, from anywhere from 4-10 minutes. It was also disclosed that the director walked by the classroom and heard "banging" on the door of the toddler classroom. The director looked inside and found the subject child in the toddler classroom, unsupervised and alone.
During today's inspection, LPA spoke with staff who were present in the indoor to outdoor transition on the date of the incident. Children interviews were not obtained, since children are infants and are non-verbal.

An exit interview was conducted, appeal right discussed, Notice of Site Visit and must remain posted for 30 days. A copy of this report was provided. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. |
