25 | On 07/19/2024 at 1:05 PM, Licensing Program Analysts (LPAs) Raymond Moorehead and Tiffanie Diep conducted a case management inspection with Tracy Bierman, Director and Ann-Marie Schoben, Assistant Director. A case management inspection is being conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on 06/26/2024 as the incident was self-reported.
The UIR documented an incident where a child reportedly did not go into their classroom from the playground during transition time. Further, it was reported that the child was sitting in the breezeway playground and was watching the school age children play. There were no injuries that were reported to occur as a result of the reported incident.
During today's visit, LPAs investigated the reported incident. Facility staff members were interviewed, documentation was obtained, a tour/census was taken, and LPAs inspected the area where the incident took place. During interviews, the following was disclosed. It was stated that the subject child was sitting at the gate that separates the school age playground from the breezeway playground, which is the pre-school's playground.
Further, LPAs observed the area in which the reported incident took place. No safety hazards were observed at this time. At the conclusion of the visit, it was determined that the child's classroom went inside their classroom without the subject child. However, it was reported that a school age teacher was present and supervising her class during this time and also was able to visually observe the subject child. It was also reported that the school age teacher knew that the subject child was near her and that when she checked on them again, she realized that their class had already went inside. Then, it was stated that the school age teacher immediately notified the front office and the subject child was walked back to their class. |
