Due to COVID-19, Licensing Program Analyst (LPA) Sharleen Robinson conducted a Tele-inspection with assistant Director, Alisa Holtegaard for the purpose of delivering findings of this complaint that was initiated April 15, 2020. LPA met with assistant Director Alisa Holtegaard, via Skype, per the director there were 45 children in care. It was alleged that a child sustained unexplained injury.

During the investigation, LPA Robinson reviewed facility documentation, conducted interviews via Skype with relevant individuals pertinent to this investigation. It was alleged that on or about March 10, 2020, a child was dropped off at the facility at approximately 8:30 am and was picked up at 3:30 pm. When the child was picked up, staff told the child’s representative, the child was "too rubbed" meaning the child had loose and/or watery bowels during the day, information obtained reported the child had diarrhea. It was also reported, the child’s representative took the child to their pediatrician on or about March 10, 2020 because the child did not want to be touched and when their diaper was taken off, the representative noticed that the child appeared to have a scratch on their buttocks. It was alleged that a staff member with long pointed fingernails scratched the child’s buttocks during diaper changes. LIC9099C for the remainder of the report

During staff interviews, LPA learned on or about March 10, 2020 a child had “diarrhea”, the child’s diaper log revealed that staff changed the child’s diaper several times that day due to “diarrhea”. On or about March 10, 2020, the child was picked up by their representative at approximately 3:30 pm, it was disclosed that staff advised the child’s representative that the child had “diarrhea” and as a result the child’s bottom was red. Staff admitted to their nails being long, however staff deny scratching the child. An internal investigation was conducted by the facility, it was disclosed that staff nails were checked for sharpness, and length. But, it was reported that although some staff fingernails were long, they did not appear to be sharp.

During other interviews with pertinent individuals, LPA learned on or about March 10, 2020 the child’s representative took the child to their pediatrician and the pediatrician reportedly diagnosed the child with a diaper rash.

There were conflicting statements regarding the child sustaining an unexplained injury. Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is deemed unsubstantiated at this time.

An exit interview was conducted via Skype, notice of site visit posted and verified via Skype. LPA Robinson provided the director with a copy of this report via email with an electronic “read receipt” . The electronic read receipt of the emailed report acknowledges receipt of this report and appeal rights. A copy of this report was emailed to the Assistant Director during this Tele-inspection on April 22, 2020.
