On 03/30/2022, Licensing Program Analyst (LPA) Rachel Zeron arrived at the facility to conduct a 10 day complaint visit initiated on 03/20/2023 concerning the above allegation. During the visit, LPA Zeron took a census of the children present, and this time met with Director Christina Lambaren to discuss the complaint allegations. Based on all the information obtained, the following is the outcome of the investigation regarding the allegation:

During the investigation, LPA made observations, conducted interviews with staff and all other relevant individuals pertinent to this investigation. It is alleged that on or about March 16, 2023 a child was found wandering on the Pre Kindergarten playground unsupervised for anywhere between 30 seconds and 5 minutes. Staff 1 (S1) was conducting circle time in the Pre Kindergarten classroom when a child that was looking out the window spotted the subject child on the playground alone. The child informed S1 and S1 went out to the playground and took the subject child back to the infant classroom. Interviews revealed that the door leading outside of the infant classroom to the playground areas was broken.

Responsible Parties would utilize that door to drop off and pick up children, the door was missing a spring and would not close properly when opened/closed. According to interviews the door had been broken since 03/08/2023, Director verified that the technician had been out on 03/28/2023 to inspect the door and LPA observed the technician out at the facility on today's date as well.

Based upon the information gathered and interviews conducted, the preponderance of evidence standard has been met, and therefore, the allegation, Staff left infant unattended in classroom is found to be SUBSTANTIATED.

See LIC 9099D for deficiency cited. A an "Immediate $500" civil penalty was assessed for Lack of Supervision. A citation for reporting requirements will be cited on a separate report, for failure to report the incident to CCLD within 10 days.

An exit interview was conducted, and a copy of this report was reviewed and provided to the Director, Christina Lambaren . Appeal rights were discussed and provided during the exit interview.

A notice of site visit was given and must remain posted for the next 30 days.

Director must have parents of all current and any newly enrolled infants within the next 12 months, complete the Parent Notification Requirements form LIC 9224, and place the completed form in the child’s facility file. A civil penalty of $100 per violation will be assessed for noncompliance.

Based on interviews conducted and observations made by LPA, child was able to leave the classroom unattended and wander to the playground where the child was found by another teacher.
This is an immediate risk to the health and safety of children in care.
