32 | Based on the interviews with all relevant parties involved, S1 observed C1 pulled themselves up on the metal gate and was holding onto the gate. S1 left the area to tend to another child and their back was turned to C1.
S1 heard C1 cry and moved in C1’s direction. S2 redirected C1 several times in the same day from holding onto the gate, S2 was on the opposite side of the metal gate when C1 fell. The staff were present in the room, C1’s fall happened in an instant, both staff heard C1 cry and reacted. Although the injury was an accident and could not have been prevented, neither staff member was positioned in the classroom to have full visibility on Child 1. Both staff provided immediate attention, the parent was notified, and C1 received further medical treatment. Therefore, the above allegation is Substantiated, meaning the Preponderance of the evidence standard has been met.
There are no deficiencies cited during this inspection. There is a Technical Violation issued on Responsibility for Providing Care and Supervision 101229(a)(1). See TV page attached to this report.
An exit interview was conducted, and a copy of this report was read, and a Notice of Site Visit, were provided to Linda Meza, Facility Representative, at the facility. A Notice of Site Visit must remain posted for 30 days. Removal of the posting is subject to a $100 civil penalty.
