On November 16, 2023, Licensing Program Analyst (LPA) Calloway met with Jeanette Honnold, Assistant Director, for the purpose of conducting an unannounced Annual/Random inspection. This Infant program is located on the right side upon entry to La Petite Academy front entrance. It is closed off with a door. Daily hours of operation are from 6:30 AM – 6:30 PM. This is the only infant room. Teacher/Child ratios are met according to Title 5 of the California Code of Regulations and Title 22 Regulations. Children enter through the front entrance. Upon arrival, LPA observed 8 infants and 2 teachers in active play and lesson. Incidental Medical Services (IMS) were discussed. The parent board was reviewed and has all the required forms posted. Fire/earthquake disaster drills (current).

LPA toured with the Assistant Director and observed six (6) additional classrooms, this Center has a school aged/Toddler component. LPA observed 71-day care children in active play/ napping, and 8 staff interacting. The facility provides breakfast, one snack, and lunch. Menus are posted. Activity schedule inside the infant room. Furniture and equipment were inspected for age appropriateness and good repair. Roster (current). Sanitation plan/log (posted). Soap dispensers have liquid soap.

LPA observed changing one table present and diapering supplies. Per Director, diaper changing is done standing (in kitchen area) paper sanitation roll (observed). Flooring (rugs) observed. Carpets (vacuumed daily) and Per Assistant Director, steam cleaning is done once a month. LPA observed enough cribs /play yards available for napping children. Bedding and linens were individually stored so that each child's bedding is identifiable, and no child's used bedding comes into contact with other bedding. The sheets and blankets are washed daily (sent home with parents to wash or staff wash after hours) and cribs are sanitized daily.The telephone service was verified (staff communicate via walkie-talkie). Heating, lighting, and ventilation are adequate. There are cubbies for children's belongings inside the room. LPA observed age-appropriate toys, books, and furniture. Drinking water is available inside the classroom in the form of a water pitcher and cups that are disinfected daily, and a label of the child’s name is placed on the cup daily. There is a fire alarms pull near the exit door and a sprinkler system inside the ceiling. There were no smoke detectors, and carbon monoxide detectors located inside the classroom. There is an appropriate amount of food and snacks. Cleaning solutions and other chemicals are kept in a cabinet to the left of the kitchen. Medicines (not provided). The day care center provides the diapers daily there is adequate supply.

LPA inspected and observed two staff sinks (clean, operable), and one small sink for infant handwashing (operable). LPA observed soap and paper towels readily available.

The water temperature is appropriate. The staff restroom is outside the room in the located on far-left wall.

Children are inspected for illnesses as they arrive. A review of medication policy indicated that prescription medication is administered only with the parent's written permission. The Director administers medication and documents the dosage, date, and time on a log. Medication is brought and taken home by the parents daily. Medication is properly labeled and stored in its original container.

There are currently no children in care with IMS needs on this site. There is a separate area for isolation and care of ill children in each classroom. They are placed in a highchair or taken to the Director’s office until the parent can pick them up.



Sign-in and out sheets were reviewed inside the classroom. Parents use a digital sign in system. The Teacher to child ratio was observed and staff names were recorded. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. Safe Sleep was discussed and LIC 9227 reviewed in child’s files.

Children's records and staff records were reviewed. LPA verified there is at least 1 staff person present with current CPR and First Aid Training (exp. 06/2024). Mandated Reporter trainings (current). The outdoor play equipment was inspected for health, safety, cushioning material, good repair, and age appropriateness. The play area has grass, wood chips, and concrete. There is an area for rest (canopy). A water pitcher and personal cups are brought outside during play. The play area was inspected and found to be free of hazards and inaccessibility to bodies of water. Separate playground schedule for preschool children. The jungle gym was not anchored. There were Little Tyke toys for the age group at play.

Mandated Reporter Training Requirements: §1596.8662 - As of January 1, 2018, childcare providers, administrators, or employees who work in a licensed facility shall complete the mandated reporter training provided according to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.
Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a daycare center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Fingerprint clearances and transfers: Before working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state-licensed facility to another using form LIC 9184
**Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit (printed out during this inspection). In addition, all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of the report documenting a Type A Citation and sign form LIC 9224 acknowledging receipt. Staff is aware of the required forms for children's files and forms that shall be posted after licensure.
**Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements
Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed childcare facility are aware of situations that present the greatest danger to children.

Per Title 22 Regulations, there are deficiencies cited during this inspection. See 809 D pages.

Exit interview conducted a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided, and left with Jeannette Honnold, Assistant Director at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days failure to maintain posting will result in $100 Civil Penalty.
