On October 4, 2023, at 09:00 a.m. Licensing Program Analyst (LPA) Kendal Zirbes met with Facility Representative Delmy Schwartzman. The purpose of the inspection was to conduct a required one year inspection at the licensed facility. LPA disclosed the purpose of the inspection to the Facility. Present during today’s inspection were 20 preschool age children, two teachers and one aide.
Indoor/Children's Area:
The preschool program is operated out of a portable located on the Hunt Elementary School campus. The facility operates two session each day an AM session, 8:30 AM - 11:30 AM and a PM session 12:30 PM - 3:30 PM.
LPA completed a tour of the classroom and observed three areas of the classroom flooring had tape placed over the edges. Per conversation with the staff the tape was applied to the areas to prevent tripping due to the flooring peeling up. Interviews confirmed the tape had been on the floor since the beginning of the 2023 school year. One area of the floor was approximately 6 inches by 24 inches and was under the tables. The second area was approximately three feet by three feet, and was under the tables and in a passageway. The third area was outside of a bathroom. Based on LPA observation, the Center placed take on the flooring to ensure the area was not a tripping hazard. However, LPA is advising the facility to monitor the flooring and ensure the classroom is in good repair and safe. An advisory notice was issued. In addition, LPA observed cans of Lysol and Spray Pak air freshener in one of the preschool bathrooms. The cans were placed on top of the paper towel dispenser that was approximately 4 feet high. LPA advised the Center to ensure that all products labeled keep out of reach of children are inaccessible at all times. LPA observed the door to the restroom was closed when the preschool children were present. When the bathroom was in use, the children were under direct supervision of a staff member. Lysol and Spray Pak was removed and placed in a locked cabinet. A technical advisory notice was issued. Additional cleaning compounds were stored in a locked cabinet. Zero poisons are stored in the classroom. LPA observed the furniture/equipment is in good condition. Classroom was free of flies, other insects, rodents. The classroom had tables and chairs to meet children’s needs.

LPA observed the classroom had designated areas for dramatic play, art, writing, manipulates, library and a computer area. In addition, there was a carpet area for story/instructional time. Children's belongings are kept in cubbies labeled with the children's names. Trash cans for solid waste have tight fitting lids. Drinking water is readily available indoors via a water cooler and cups. All materials and surfaces are toxic free, window screens in good repair and free of debris. There is a carbon monoxide detector, smoke detector and Fire Extinguisher in the classroom. There are two bathrooms located within the classroom which were equipped with a toilet and sink. Bathrooms were observed to be in safe and sanitary operating condition. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. There was working telephone service within the classroom. There is an isolation area for children who become ill. The isolation area is located within the nurses office located in the main office of the elementary school. The facility maintains a comfortable temperature at all times. LPA observed the centers electronic sign in/out sheets, which are completed daily. Smoking is prohibited on the premises. LPA did not observe prohibited childcare items during this inspection. Electrical outlets are inaccessible. Firearms/weapons are not allowed or stored on premises. Zero bodies of water are on the premise. Meals are provided through the schools food program.
Napping
The preschool program only offers a half day program. Therefore, the children do not nap at the facility. The classroom has a designated areas for relaxation and quite play.
Outdoor
LPA inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness. The outdoor play space is shared with the kindergarten class on Hunt Elementary. LPA advised the facility representative to supervision the children as they walk from the classroom to the enclosed outdoor play space. The outdoor play space has one large play structure, tricycles, hula hoops, tables/benches. A tree provides shaded area. There was cushioning material under the play structure and the play structure was securely anchored to the ground. LPA observed one of the plastic arms of the basketball hoop in the play space was broken was tilting to one side. The basketball hoop was secure at the time of this inspection, however LPA advised the Center to monitor and fix the basketball hoop.

Health Related Services
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Staff/Personnel Records
LPA reviewed five child record and three Personnel files during this inspection. Staff and child files were complete. A staff person was on site with current CPR/First Aid. A technical assistance was issued regarding CPR/First Aid certification.

Facility Records


LPA and Facility representative reviewed the LIC 311A Records To Be Maintained At The Facility. LPA observed a current roster, disaster drill log (last drill September 2023). LPA advised the facility to add the physicians name and number to the roster.

Posting Requirements
The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393), menu, car seat law (PUB 269), emergency disaster plan, earthquake preparedness checklist.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The Center shall permit the Department to inspect the child care center and to privately interview children or staff, to determine compliance with or to prevent violations of child care laws or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Facility representative was advised of the requirement to report Unusual Incidents. Facility representative informed to utilize the Unusual Incident Report/Injury Report LIC624 when submitting the report to the department (www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. The On-Duty Worker is available for questions at (661) 202-3318 Monday through Friday 8am-5pm.

Based on LPA observation, four advisory notices were issued during this inspection.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Delmy Schwartzman.
