On 11/21/2024 Licensing Program Analyst (LPA), Isabel Ortega met with Martha Osita, Lead Teacher and conducted an Annual Random Inspection. LPA toured and inspected the facility in accordance with the facility sketch. During inspection LPA observed 14 children in care and three Staff providing care and supervision. Facility operates in one classroom referred to as the Preschool, serving ages 3 years to 5-year-olds. The Preschool Center is located on SBCUSD Wilson Elementary School premises.

Facility operates in two sessions Monday through Friday; The first session is from 8:00 a.m. to 11:00 a.m. and the second session is from 12:00 p.m. to 3:00 p.m.

An isolation area was inspected, which takes place in the in the classroom in a designated area away from the remaining of the children. Children have the option for rest time and quiet time if needed. Age-appropriate sinks and toilets were inspected for availability and good repair. Each restroom has adequate toilet paper, hand soap and paper towels available. Restrooms are cleaned, restocked of toiletries, disinfects, sanitizes daily by on-site custodian.

According to Staff, there are no bodies of water on the premises. LPA did not observe any bodies of water during this inspection. According to Staff there are no weapons nor guns at the facility. LPA did not observe any weapons or guns at the facility during inspection.

The furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and AC ventilation were evaluated. LPA observed individual storage for children's belongings with names labeled. Facility sampled water for lead, according to facility Staff children are provided with individual disposable water bottles which are refilled with filtered water from a a water dispenser (individual water bottles and 5 gallon water jug were observed. The facility provides individual disposable cups as needed.

Facility has all the required posted documents: Facility License, Notice of Parent's Rights Poster, Emergency Disaster Plan, Car seat information, and Earthquake Preparedness Checklist.

The facility has a First Aid kit with a thermometer in a Blue backpack. The blue backpack is taken during outdoor play. The smoke detectors, carbon monoxide were observed to be operating properly. The fire extinguisher (2A10BC) is reading in green and meets the Fire Marshal codes and standards. The Fire extinguisher was last serviced on 11/24/2023 (LPA observed a yellow tag with date hole punched).

Trash cans with tight lids were observed. Snacks were inspected, and it was properly labeled, stored, and within expiration date. Cleaning supplies are out of reach of children. Emergency Drills are conducted every sixth month last drill was conducted and documented on 10/17/2024 at 10:18 a.m.

Food allergies are noted and are posted in the snack area and in each classroom as needed. The snack menu is posted in the main office and parents may request a tangible copy per request. Also the menu is posted once a month on Class Dojo.



The outdoor play area was inspected and observed to be free of hazards, loose, or sharp objects. Equipment was inspected for safety, cushioning material, good repair.

Children's records were reviewed for completeness. Health History, Emergency contact and Medical Exams; Immunization Records are all in the children's file. According to Staff a facility roster is up to date.

Staff are currently certified in BLS Provider CPR, which does not expire until August 2025. Facility was made aware this certification is not the required Pediatric required for Childcare Program another staff stated her CPR was on line demonstrated card, no other staff have a valid Pediatric CPR (a Type B citation has been issued). Facility will need to enroll and pass the correct Pediatric or EMSA approved CPR/First Aid.

Childcare Provider Mandated Reporter (AB1207) training certificate is required every two years www.mandatedreporterca.com. A technical Violation has been issued according to documentation staff completed the incorrect training School Personnel.

The following Incidental Medical Services (IMS) were discussed.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Upon hired date all staff are required to be immunized and show proof of immunization records according to Title 22 regulations.

Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. New employees shall have 90 days from date of employment to complete training as required. The training may be conducted at the following website www.mandatedreporterca.com.

According to facility documentation all staff are aware they are mandated child abuse reporters and have the responsibility of reporting any suspected child abuse to the Child Abuse Hot line at (800) 540-4000.

Per Title 22 Regulations facility is complying with rules and regulation, and no deficiencies will be issued today. Technical violations were given today as a form of presentation. An exit Interview was conducted with Lead Teacher, Martha Osita.

A copy of this Report, a Notice of Site Visit and appeal rights were provided on this day.
