25 | On 5/17/2022 at 10:15 am , Licensing Program Analyst (LPA) Patricia Berry conducted a case management incident investigation. LPA was granted access into the facility and met with director. LPA took a census. LPA interviewed staff and children.
Regarding an Unusual Incident Report, self reported by director to Community Care Licensing on 4/29/2022 it was alleged a teacher inappropriately handled a child. Director stated the parent called the police department and officer Mason arrived to talk with both parent and child. Director stated the officer determined the child had conflicting stories and the parent decided to keep child in the facility but requested that the child be put in same classroom with the sibling. Children interviews revealed conflicting information.
Based on information obtained by director and determination from the officer there does not appear to be a violation of Title 22 regulations at this time. LPA attempted to obtain police report from the Rancho Cucamonga Police Department 5/6/22.
Exit interview with director, report, appeal rights and Notice of Site Visit provided to director.
LPA observed director post notice. |
