25 | On 09/09/2021 at 3:45pm, Licensing Program Analysts (LPAs) Destinee Hogue and Laura Mejorado conducted an unannounced case management inspection to follow-up on an Unusual Incident Report (UIR) submitted by the facility to the Riverside Child Care Regional Office via email on 07/19/2021. During this inspection, LPA Hogue toured the facility inside and outside, took census of children present on this date, and verified facility associations. At the time of this inspection, there were no infant's present at the facility. Upon LPAs arrival, LPAs met with Interim EHS Director, Deborah Gunn and Health Manager, Denise Sinohui. At approximately 4:22pm, Center Coordinator, Gloria Delgado arrived at the facility. LPAs met and discussed the following with Ms. Delgado, Ms. Gunn, and Ms. Sinohui:
On Monday, July 19, 2021 at approximately 7:00am, staff arrived at the facility and observed smoke coming from the back playground. Staff approached the back playground and observed the playground wood chip box on the infant playground (far right side of the outdoor play area) was on fire. At this time, staff also observed property damage to the infant playground fence. The fire damaged the shading canopy, plastic fencing, and wood chips, located on the surface of the infant playground. Children were not present at the time the fire was observed and parents were notified via telephone that the facility will be closed till 07/20/2021. Facility reopened on 07/20/2021. Staff immediately contacted 911, who then dispatched City of San Bernardino Fire Department.
During this inspection, LPAs toured the infant playground and inspected the areas that were affected by the fire. Per statement from Ms. Gunn, the materials for the fencing and canopy are on back order and at this time, the facility does not have a date of completion for the repairs on the infant playground. Per statement from Ms. Delgado, the infants (Birth-12 months) are currently utilizing preschool "baby buggies," and staff are pushing infants around the preschool playground, in the "baby buggies." |
