25 | On 02/01/2023 at 02:32PM, Licensing Program Analysts (LPAs) Perla Ordones and Susan Brewer arrived at the facility unannounced for the purpose of conducting a case management inspection. LPAs were greeted by Assistant Director Monique Abeyta, granted entry to tour the facility, and conducted a census. During a separate visit conducted on 12/29/2022, a staff member identified as a floater was interviewed by LPAs. At the time of the interview, the staff in question was not observed to be working in the preschool classrooms. However, on 02/01/2023 while LPAs were reviewing the facility’s Guardian roster for pertinent information, it was observed that the staff member in question was not actively associated to the facility though they did have active criminal record clearance. Based on LPAs observations made and review of facility records, it was determined that the facility failed to meet the criminal record clearance requirements per Title 22 regulations.

A civil penalty of $500 was assessed for the Type A deficiency. The civil penalty will continue to accrue $100 per day until the deficiency is corrected. See LIC421FC for Civil Penalty assessment.
LPAs also informed the Assistant Director to provide a copy of this licensing report that documents a Type A citation to parent/guardians of all children currently enrolled by the next business day or the next day children are in care and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed acknowledgement of Receipt of Licensing Report form (LIC9224) must be placed in the children's file for verification.
Exit interview conducted and report was reviewed with Assistant Director Monique Abeyta. Appeal rights were provided. The Notice of Site Visit (LIC9213) was posted where the parent/guardian of children enter and exit the facility. The Notice of Site Visit (LIC9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
