32 | next changing at approximately 2:15pm which is three hours after the child(ren)'s last diaper change. The child(ren) received their last changing at approximately 4:00pm prior to checking out of the facility.
On or about November 8, 2021, there was approximately four children present in the classroom. According to interviews with pertinent parties, a staff member entered the infant classroom to relieve another staff member for lunch. According to interviews, the child(ren) in question was napping when the staff entered the classroom. According to disclosures, the children in the classroom appeared to be crying more than usual and napping children were beginning to wake up from their naps. As each child woke up, the staff member in the classroom placed the children in the facility 'buggy" cart and wheeled the children outside, as an attempt to soothe the crying infants. Staff was pushing the children in the "buggy" cart outside until the second staff returned from lunch.
When the second staff returned from lunch, the children remained outside for approximately 15 to 20 minutes, then were brought inside the facility. When the staff member and children returned to the classroom, staff started removing children from "buggy" cart. Staff noticed the child(ren) in question did not have a diaper change within two hours and began changing the child(ren). Staff documented the child(ren)'s diaper change, however staff did not include whether the child(ren) had a rash or if rash ointment was applied. According to interview disclosures, during pick up around 4:22pm, the child's authorized representative arrived and was notified the child(ren) had a diaper rash because the child(ren) was left in his/her diaper for approximately three hours.
Based on interviews with pertinent parties, records reviewed, and interview disclosures, a child(ren) was left in his/her diaper for approximately three hours and a staff member observed a diaper rash on the child earlier in the day. According to interview disclosures, staff are required to check a child's diaper every two hours and staff are required to change a child every two hours whether a child has a wet or soiled diaper. Based on this information, the department has determined the preponderance of evidence standard has been met, the above allegation is found to be SUBSTANTIATED
