25 | On 04/22/2024 Licensing Program Analyst (LPA) Justin Giese conducted an unannounced Case Management visit for the purpose of inspecting newly installed playground equipment and substrate located on playground #4. LPA met with Facility Administrator, Laura Gallegos, and discussed the following.
On 02/26/2024 Facility submitted an Unusual Incident Report (UIR) which stated playground #4 would temporarily be closed from 03/06/2024 to 03/15/2024 for construction. The playground will be resurfaced and a new play structure installed.
On 03/05/2024 LPA made an unannounced visit to the facility for the purpose of following-up on submission of UIR. At time of visit LPA was informed that due to planned construction for playground #4 the facility has began a rotation schedule for preschool aged children to use playgrounds 2-3. LPA obtained playground rotation schedule as well as renderings/drawings for planned construction. Due to weather and other delays the playground's construction took longer than expected and was finalized on 04/18/2024.
At time of today's inspection, LPA toured playground #4 and noted the following: Play structure is firmly anchored to the ground, a large canopy provides shade, adequate cushioning material (wood chips) are available under play structure/fall zones. LPA did not observe any health or safety concerns while touring the playground or facility. No deficiencies have been cited.
An exit interview was conducted, LPA Giese provided Facility Administrator, Laura Gallegos with a copy of this report and a notice of site visit.
