On 10/28/2024 Licensing Program Analyst (LPA) Justin Giese made an unannounced visit to the facility for the purpose of following-up on the submission of an Unusual Incident Report (UIR) which was submitted by the facility to the Riverside Regional Office on 10/01/2024. LPA met with Facility Director, Stephanie Castro and recorded the following information.

According to the UIR, it was reported a Child’s Representative spoke to facility staff on 10/01/2024 and wanted to address a potential personal rights violation. It was alleged a child informed their representative a staff member had grabbed their arm. In receipt to this information, the Facility conducted an internal investigation into this matter and collected written statements/emails from pertinent individuals, these documents were included with the submission of the UIR.

On 10/22/2024 LPA conducted a site visit and toured the classroom where the alleged incident occurred. LPA conducted interviews with Facility Director, staff, and pertinent individuals involved. LPA made an additional unannounced visit on 10/28/2024 to conduct further staff interviews. LPA did not record any corroborating statements from individuals that support a potential personal rights violation had occurred.

There appear to be no Title 22 deficiencies observed at this time. The facility took the appropriate steps to record this incident via submission of UIR and document statements and events from its own internal investigation into this matter.

An exit interview was conducted, and report reviewed with Facility Director, Stephanie Castro on this date, 10/28/2024.

A notice of Site Visit was issued, Facility Director was instructed to display it in a prominent location of the facility for 30 days.
