On date and time listed, Licensing Program Analysts (LPAs) Taityana Benson and Elyse Jones arrived at the facility to conduct a required/annual inspection as part of a compliance review. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed:
Normal days and hours of operation are: Monday – Friday, 6:00 a.m. – 6:00 p.m.

OFF-LIMIT AREAS INCLUDE: Entire 2nd Floor, Back Yard-Right Side Yard (when facing the house) , and Garage

The facility is operating within the licensed capacity and appropriate ratios


· Appropriate supervision provided during this inspection
· A working telephone is present and current number on file
· Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection.
· All hazardous items are stored inaccessible to children
· Toxins are locked
· Weapons are not present according to Licensee, Agnes Cristobal. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations
· Stairs are barricaded
· Verification of control of property on file (Mortgage Statement)
· Facility Sketch, Emergency Disaster Plan & Notification of Parent’s Rights poster are posted
· Mandated Reporter Training expires on 08/18/2024
· Pediatric CPR and First Aid Card expired on 03/06/2023

Based on the record review and facility tour, the Licensee did not meet the above regulation which poses an immediate Health and Safety risk to the children in care. During the facility tour the LPAs observed a child in an area that the Licensee designated as “off limits” after the last annual and did not notify the department. LPAs observed medication, biohazard items, oxygen machines and other hazardous items that should be made inaccessible to the children in care.

During the facility tour the LPA observed dog feces in the children’s outdoor play area. The Licensee stated the children have not played in the back yard today. However, the LPA observed an excessive amount that was and was not fresh.

Safety precautions shall include but not limited to: (9) Each family child care home shall have a written disaster plan of action prepared on a form approved by the Department. All children, age and ability permitting, and the provider, the assistant provider, and other members of the household, shall be instructed in their duties under the disaster plan. As their age and ability permit, newly enrolled children shall be informed promptly of their duties as required in the plan. (A) Each family child care home shall conduct fire drills and disaster drills at least once every six months.

· No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.

- LPA discussed the safe sleep regulations with Licensee, Agnes Cristobal and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

- LPA also informed Licensee, Agnes Cristobal of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

- Licensee, Agnes Cristobal was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Also, LPA Taityana Benson informed the Licensee, Agnes Cristobal to provide a copy of this licensing report dated 06/27/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

The Licensee, Agnes Cristobal, confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.
