25 | On 07/12/23 at 3:25 p.m., Licensing Program Analyst (LPA) Blanca Ruiz arrived at the facility to follow up on an incident learned during another inspection, related to another matter. Involved parties are aware of the incident. LPA met with Assistant Director, Maria Rico. During today’s inspection, the facility was toured, and census was taken. LPA observed teacher to child ratio and staff and children’s interactions.
During the 07/03/2023 inspection, it was learned that on or about the first week of 06/2023 a parent/legal guardian visited the facility to oversee child(ren)’s daytime activities, specifically lunch time since their child was allegedly losing weight lately and it was assumed, it had to do with the facility's lunch options/portions. The facility serves age groups from Transitional Kindergarten (TK ) to Pre-Kindergarten (Pre-K). It was explained by staff that at lunch time children’s plates are served with small portions according to age, and bigger portions are served for older children. However, if smaller children ask for seconds, they are served more. Seating is arranged according to age; however, on the day in question, Child #1 was mistakenly seated with the older group’s table and their plate was retrieved, the child was then given a smaller portion. The parent/legal guardian present asked for an explanation as to the reason for the retrieval of their child’s plate and demanded a reason. It was explained, but the parent was dissatisfied with the answer, so the parent removed their child from the facility, took their child's sign in/sign out sheet without the center's permission, and used profanity in the presence of staff and children in care. The parent/legal guardian was removed from the classroom as children were napping and taken to the director's office to try to resolve the misunderstanding; however, the parent/legal guardian verbally threaten to physically harm staff, but due to the fact that their child was present, the parent decided to leave the facility.
Licensee stated they forgot about reporting this incident to Community Care Licensing (CCL). Therefore, licensee failed to report an unusual incident(s) that occurred at the facility which potentially jeopardized the health and safety of the children in care. Licensee was reminded to be in continuous communication with Community Care Licensing (CCL) related to any questions, concerns and/or to verify substantial compliance.
