Licensing Program Analyst (LPA) Maddox met with Site Supervisor Mrs. Clementina Baxter today for the purpose of conducting an unannounced Annual/Random inspection. Upon arrival, LPA observed 2 children present with 2 teachers. This preschool program operates on the grounds of Rio Vista Elementary school and occupies 1 modular classroom (PS26) for children ages 3 to 5 Yrs. This site has 2 sessions: (AM) 8:00AM to 11:00 AM; PM sessions starts at 12:00 Noon to 3:00PM, because of the half day schedules, children do not nap.

LPA observed toys, materials, furniture and equipment were age appropriate and in good repair. Telephone service was verified; Heating, lighting, and ventilation are adequate; cubbies are available for children to store their belongings; Drinking water is available inside the classrooms in the form of Water cooler with disposable cups. LPA observed classrooms with smoke detector, carbon monoxide detector and fire extinguisher. Flooring and carpets were observed to be clean and in good repair.

LPA inspected and observed 2 clean bathrooms for children (each with 1 toilet and 1 sink) and 1 Staff bathroom. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available.

Outdoor play equipment was inspected for health, safety, cushioning material(center utilizes rubber matting for cushioning material), good repair and age appropriateness. Large play equipment and structures are securely anchored. The area was observed to be free of debris. There is an area for shade and rest. There is a water fountain for drinking water. Play area was inspected for safety hazards and inaccessibility to bodies of water.

Children are inspected for illnesses (wellness policy) as they arrive. A review of medication policy indicated that prescription medication is administered only with parent's written permission. The Site Supervisor and/or Teacher administers medication and documents the dosage, date and time onto a log. Medication must be properly labeled and stored in its original container. There is a separate area for isolation and care of ill children in the elementary school office.

Staff files are kept at the San Bernardino School District Office. There were 2 children present during this inspection, it was the first day for 1 child and Director states the file for the 2nd child has not been sent from the School District. ID and Emergency information was available. Director has current CPR and First Aid (exp 8/2023) and current Mandated Reporter Training (exp 11/2023).

Meals are prepared at the main Elementary campus cafeteria, teachers retrieve lunch carts to bring to classroom. Menus are posted in each classroom. Allergy lists are posted in each classroom.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Teacher child ratios were observed and staff names recorded (2C/2T). Care and supervision was evaluated to determine if the basic needs of children are met and appropriate.

This site utilizes electronic Sign in and out sheets. LPA observed Licensing and Parent boards to be incompliance including the required posting (menu, daily schedule, community activities). Fire/earthquake drills current. First Aid Kit observed.

LPA requesting updated LIC 308 for this site to include name of Lead Teachers, also when a teacher is out for an extended period, please update LIC 500 and keep file on site for Substitute teacher,

There were no violations noted as a result of this inspection, Center is operating in accordance to Title 22 Regulation. Exit interview conducted and a copy of this report was left at the facility. A copy of this report must be made available to the public for 3 years.
