25 | On 1/27/2022 at 11:25 am, Licensing Program Analyst (LPA), Patricia Berry conducted a case management incident investigation regarding an Unusual Incident Report reported on 12/22/21.
It was reported while playing a game called “save me” a child’s arm was fractured. It was reported the child was on the top of the slide, getting ready to slide down and another child came and held the child's arm to “save the child”. LPA interviewed children, staff and parent. During children interviews it was discovered the children were playing a game called "save me”. A child would slide down the slide and another child would try and save the child before the child gets completely down the slide. Children interviews revealed child #1 (C1) was at the top of the slide getting ready to slide down and shouted, "save me" and child #2 (C2) went to save C1 as C1 started sliding down the slide. C2 grabbed ahold of the C1’s arm. C1 slide down the slide and when C1 was down the slide, C1 told the teacher what happened. During children interviews it was revealed the teacher was standing right by the slide when the incident occurred. Staff interviews revealed the children were playing a game, and C2 tried to save C1 at the top of the slide. C2 held on to C1’s arm as C1 was trying to slide down the slide. Staff revealed C1 told the teacher what happened, the teacher assessed C1 and there did not seem to be any first aid needed at the time. Staff stated C1’s caregiver came for C1 approximately eight minutes later. Staff stated they learned the next day C1 had an arm fracture. Parent interviewed revealed the incident to be an accident. The parent expressed the love for the school and knows their children are well taken care of. Parent stated the children were playing a game and that is how the incident occurred.
