13 | On January 12, 2023 Licensing Program Analyst (LPA) Elyse Jones arrived at the facility to conclude the investigation and deliver findings regarding the above allegations. LPA met with Director, Mona Salazar, at the time of inspection and stated the purpose of the inspection. LPA conducted a tour of the facility inside & outside. Interviews were conducted with pertinent parties.
On October 26, 2022 a complaint was received alleging staff at the facility violated a child(ren) personal rights. It was reported staff handled a child roughly. It was noted while watching the cameras the Reporting Party (RP) observed a staff member “snatch” a cup from a child and staff “grabbed the child by her arms and began shaking” a child. During interviews with pertinent parties, it was disclosed that staff have never observed any other staff acting inappropriately or violating the Personal Rights of children in care. Although the incidents were observed it was disclosed that the intent of how the staff handled each incident was unclear. Children were too young to interview and video footage could not be obtained for LPAs to review. |
