On 6/13/24, LPA Diaz conducted an unannounced subsequent complaint investigation for a complaint received at Palmdale RO on 5/16/24. LPA met with Center Director, Micha Hendrix, who granted LPA access and authorized LPA to complete a safety inspection. Upon arrival, LPA observed 28 children in care with 3 staff. LPA conducted a safety inspection and found zero deficiencies.

During this inspection, LPA conducted record reviews for 4 children (2 - 5mos, 1 - 2yo, 1 - 1 year 5 mos)
in care. LPA completed confidential interviews with 7 staff (S1 and S7), Children in this age group were too young for interviews as they are unable to understand and answer LPAs questions. The allegation is Staff did not notify child’s authorized representative of child’s injury.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the above allegation is UNSUBSTANTIATED.
