On Wednesday October 1, 2025, at 1:00 PM, Licensing Program Analyst (LPA) Giovanni Cristales arrived at the above facility and met with Director, Katrina Henderson, and conducted an unannounced annual/required Inspection for the Infant center with a capacity of 24 children. LPA toured and inspected the facility in accordance with the facility sketch. Upon arrival, LPA observed one (1) classroom with three (3) staff, and eight (8) infant children. There were no school-aged children present at the time of the visit. The facility is also licensed for the School Age (364819503) and Preschool program (364819502) components. The facility operates Monday through Friday from 6:00 AM to 6:30 PM. Incidental Medical Services (IMS) was discussed.

Indoor/Children’s Area: Indoor activity space for infants is physically separate from space used by children in the preschool and school-age childcare center components. The Infant center is equipped with appropriate furniture and equipment including cribs (6), and mats, changing tables (2) and feeding chairs (3). Furniture is maintained in good repair and safe condition (age appropriate). Toys are safe and LPA did not observe sharp points or edges, or small parts that could be swallowed. Per Director equipment, toys and furniture are sanitized daily. There are two hand washing sinks within the arm’s length of the changing table. There is another sink for bottle cleaning or used as needed. No infant is left without supervision (visual), nor left unattended. The facility is in compliance with the staff/infant ratio and is not exceeding the approved capacity for the facility.

The Infant Center is clean, safe, sanitary and in good repair, there are no potentially hazardous areas observed (passageways), floors are clean and safe, cleaning compounds are inaccessible, poisons locked, furniture and equipment in good condition, play equipment are age appropriate. Each infant has its own cubby, trash cans have tight fitting lids, drinking water is available, no toxic materials observed, no fireplace in the room, no window screens.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA observed carbon monoxide, smoke detectors, and Fire Extinguisher (3/24/2025). There is a working telephone (Portable) on the premises, an isolation area for ill children at the office area, room temperature is comfortable, First Aid Kit observed (located inside room with cribs), sign in/out observed, no smoking on the premises, no baby walkers or other prohibited play equipment on the premises.

Napping: There are mats for infants that can climb out of a crib. Each crib is used for one infant at a time, bedding changed daily or as needed, cribs and cots are arranged to allow staff to reach an infant without having to step over an infant. The changing table is padded with washable plastic, in good repair. LPA observed cots in safe condition, sanitize after each use, cots spaced to have access to a safe walkway, bedding is not in contact with other bedding.

Infant Needs and Service Plan: LPA observed the Infant Needs and Service Plan in infant file (updated quarterly).
Infant Food Service: LPA observed the individual feeding plan for infants, highchairs and seating equipment in good repair. Director was reminded when food brought from home, it shall be labeled with an infant name and date.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Outdoor: Activity space is physically separate from the space used by children in the preschool and school-age center component. The outdoor space is maintained in a safe condition and is free of hazards, cushion material to absorb fall. The center has a shaded area, playground free from hazards and debris, play structure has cushioning material, sandbox inspected daily, air condition unit is Inaccessible with net sturdy covers.

Health Related Services:Per Administrator, only diaper rash medication is used. LPA observed food allergies to be posted.

Food Service: Food prepared and served in a safe manner, food preparation is completed in the kitchen, cleaning compounds are kept in the supply room (off limits), refrigerator temperature in regulation, menu posted in kitchen and classroom, breakfast, snack and lunch are provided and as the infant feeding is necessary.

Documents Discussed: IMS (Incidental Medical Services).
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Staff and infant files were reviewed and documented. Medication – all medications are stored with child’s name and date. Sign in/out sheets are complete.

Staff Records: LPA reviewed files for staff Qualifications, immunization, licensing forms (LIC9108, Mandated Reporter expires 06/24/2026, LIC508, LIC 9052, LIC 503).

Facility Records: LPA observed the Roster to be complete and current, staff has current CPR/First Aid expires 09/11/2026, fire and earthquake drills are conducted 09/12/2025.

Posting Requirements: Center has required postings, (License, Facility Sketch, Menus, LIC 613 Personal Rights, PUB 269 Child passenger restraint system poster, PUB 393 Notification of parent rights, LIC 610 Emergency disaster plan, LIC 9148 Earthquake preparedness, lead poison). LPA Observed Lead Water Testing Results.

The director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The Licensee shall permit the Department to inspect the family childcare home, and to privately interview children or staff, to determine compliance with or to prevent violations of family childcare laws or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.



No deficiencies were observed in today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Katrina Henderson.
