This report is being amended from its original version created on 7/10/2026 with the purpose to correct the day and date on the opening paragraph.

On Friday July 10, 2026, at 8:45 AM, Licensing Program Analyst (LPA) Giovanni Cristales arrived at the above facility and met with Director, Katrina Henderson, and conducted an unannounced annual Inspection for the Infant component with a capacity of 24 children. LPA toured and inspected the facility in accordance with the facility sketch to ensure the center follows Community Care Licensing Title 22 Regulations. Upon arrival, LPA observed one (1) classroom with three (2) staff, and six (6) infant children. The facility is following the staff/infant ratio and is not exceeding the approved capacity for the facility. The facility is also licensed for the Pre-school program (364819502), and School-Age (364819503) components. There were no school-aged children present at the time of the visit. All staff have been fingerprinted and are associated with the facility license number. The facility operates Monday through Friday from 6:00 AM to 6:30 PM.

Indoor/Children’s Area: Indoor activity space for infants is physically separate from space used by children in the preschool and school-age childcare center components. The infant room has its own separate entrance. The Infant center is equipped with appropriate furniture and equipment including cribs (6), and mats, changing tables (2) and feeding chairs. Furniture is maintained in good repair and safe condition. Per Director equipment, toys and furniture are sanitized daily. The room is separated into two sections. One section is for infants who are mobile, and the other section are for children who are not mobile. There are two hand washing sinks within the arm’s length of the changing table. There is another sink for bottle cleaning or used as needed. No infant is left without supervision (visual), nor left unattended.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The Infant Center is clean, safe, sanitary and in good repair, there are no potentially hazardous areas observed (passageways), floors are clean and safe, cleaning compounds are inaccessible, poisons locked, furniture and equipment in good condition, play equipment are age appropriate. Toys are safe and LPA did not observe sharp points or edges, or small parts that could be swallowed. Each infant has its own cubby, trash cans have tight fitting lids, drinking water is available (dispenser), no toxic materials observed, no fireplace in the room, no window screens. There is a working telephone (Portable) on the premises, an isolation area for ill children at the office area, room temperature is comfortable.

Napping: There are mats, cots for infants that can climb out of a crib. Each crib is used for one infant at a time, bedding changes daily or as needed, cribs and mats are arranged to allow staff to reach an infant without having to step over an infant. The changing table is padded with washable plastic surface, and in good repair. LPA observed mats, cots in safe condition, sanitize after each use, cots spaced to have access to a safe walkway, bedding is not in contact with other bedding.



Infant Needs and Service Plan: LPA observed the Infant Needs and Service Plan in infant files.
Infant Food Service: LPA observed the individual feeding plan for infants, highchairs and seating equipment in good repair. Director was reminded when food brought from home, it shall be labeled with an infant name and date.

Food Service: Food prepared and served in a safe manner, food preparation is completed in the kitchen, cleaning compounds are kept in the supply room (off limits), refrigerator temperature in regulation, menu posted in kitchen and classroom, breakfast, snack and lunch are provided and as the infant feeding is necessary. The facility has a clean refrigerator’s/freezer’s and all item’s expiration dates are current. LPA observed food allergies to be posted.

Outdoor: Toured and Inspected. The play area is fully fenced (chain link). Activity space is physically separate from the space used by children in the preschool and school-age center component. The outdoor space is maintained in a safe condition and is free of hazards, cushion material to absorb fall. The center has a sail shaded area, playground free from hazards and debris, play structure is inspected daily, air condition unit is Inaccessible with sturdy mesh cover.



Health Related Services: Per Administrator, only diaper rash medication is used.

There are no window cords accessible to children. First Aid Kit was observed to be full and accessible (thermometer, bandages, gauze, scissors/tweezers, gloves). LPA observed sign in and out sheets electronically (ProCare) and roster personnel to be current. Per director, transportation is provided, and there is one child who requires IMS.

Posting Requirements: Failure to comply with posting requirements shall result in an immediate civil penalty. The parent board was reviewed and has all the required forms posted. The following were observed posted as required: Facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393L), menus, car seat law (PUB 269), emergency disaster plan, earthquake preparedness checklist.

Staff/Personnel Records: Staff file review reveals staff are qualified for their perspective positions. LPA reviewed one (1) staff file, and six (6) infant files. CPR/First Aid, TB clearance, health screening, criminal record statement, statement acknowledging suspected child abuse, and employee rights were observed in each staff’s file. Staff #1 CPR/First Aid certificate, Expires: 3/15/27, Mandated Reporter Expires: 3/8/27.



The following general information was discussed during this inspection:

The director was advised of the requirement to report Unusual Incidents. The licensee was informed to utilize the Unusual Incident Report/Injury Report LIC624 when submitting the report to the department (email address on the website: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of the daycare center. In addition, a written report shall be submitted to the department within seven (7) days following the occurrence of any events specified above.

All staff are required to take 3-hour Child Care Provider Mandated Reporter training (AB1207) every 2 years. Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors, and employees to complete training as specified in their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition of licensure. New employees shall have 90 days from the date of employment to complete training as required. The director is aware self, and all staff are mandated child abuse reporters and have the responsibility of reporting any suspected child abuse to the Child Abuse Hot-line at (800) 540-4000. For additional information and forms visit our website at: www.cdss.ca.gov.

Criminal Record Clearance- CCC - Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Lead Testing- CCC Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

Per Director, facility’s water is not used for drinking or food preparation. Facility purchases water from water company (Master's Refreshment Sevices, Account #301690). LPA observed and collected the purchase receipt, Last statement: 6/01/26.

Safe Sleep - CCC LPA discussed the safe sleep regulations with the facility representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep,as an additional resource. LPA also informed facility representative of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS)- CCC The facility provides Incidental Medical Services (IMS).
