On July 1, 2026, Licensing Program Analyst (LPA) Annelise Villa conducted a follow up complaint investigation related to the allegations above and to deliver findings. LPA disclosed the purpose of the investigation and was granted entry into the facility by assistant Blanca Madrigal-Rodriguez. Alma Sanchez, Licensee arrived during this visit. A tour of the facility was conducted. LPA verified a census of 8 children in care with 2 staff, including Licensee working under the facility license.

Allegation #1 states inadequate supervision resulted in daycare children cutting their own hair and clothing while in care. According to interviews, on April 2, 2026, Licensee stated the daycare children had been left under the supervision of Staff #1. Interviews conducted during the investigation revealed the children had been left unsupervised in the daycare area.

The investigation further determined the scissors available to the children were crafting scissors, rather than child- safe scissors. LPA observed the scissors were sharp enough to cut hair and fabric, making them inappropriate for unsupervised use by daycare children. Throughout the investigation, the Licensee provided conflicting statements regarding how the scissors came to be in the daycare area, and the origin of the scissors could not be conclusively determined. Based on interviews, observations, and the information gathered during the investigation, it can be determined Licensee and Staff #1 did not ensure the supervision necessary. Therefore, the allegation is substantiated, this is a Type A citation. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met.

Allegation #2 states a lack of supervision resulted in daycare child exiting the van. This incident was self-reported by Licensee on March 2, 2026. Unusual Incident report made by Licensee stated multiple children were exiting the vehicle and Child #1 exited at the wrong school mistakenly. Both parent and Licensee stated Licensee had been made aware Child #1 switched schools in August 2025. Child #1 exited the vehicle near the bus drop off zone and began to walk to the correct school until being stopped by an employee approximately half a mile from where they were dropped off. Licensee did not ensure Child #1 was taken to the correct school which resulted in Child #1 exiting Licensee’s van at an incorrect location. Therefore, the allegation is substantiated, this is a Type A citation. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met.

Upon Receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.

Exit interview conducted with Licensee. A copy of this report, appeal rights and Notice of Site Visit were left with the director.

Allegation states Licensee did not prevent adult from grabbing and yelling at daycare child.During the investigation, LPA conducted interviews with Licensee, staff, and children. Interviews with relevant complaint parties revealed conflicting statements and the allegation could not be corroborated. Therefore, the allegation has been found unsubstantiated. Licensee denies the allegations and stated her daughter was never alone with children and no punishments are given to daycare children.

Exit interview conducted with Licensee. A copy of this report, appeal rights and Notice of Site Visit were left with the director.
