On 04/01/2022 at 10:57 am, Licensing Program Analyst (LPA) Patricia Berry conducted a subsequent complaint investigation to deliver final findings. LPA was granted access into the facility. LPA met with Tanina Smith/licensee toured facility and took a census.

It was alleged day-care children are left unattended in a vehicle while staff pick up/drop off children at school. During the investigation, LPA Berry Interviewed all pertinent parties.
Licensee stated there has been times when she left her own children in a vehicle when picking up and dropping off from school however, she has never left day-care children unattended.

Based on interviews conducted, there is conflicting information on whether day-care children are left unattended, did or did not occur, therefore allegation Day care children left unattended is unsubstantiated..

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

Exit interview conducted with licensee, report, appeal rights and Notice of Site Visit provided to licensee.

LPA observed licensee post Notice of Site Visit. Notice of Site visit must be posted for 30 days.
