On this date and time, Licensing Program Analyst (LPA) Taityana Benson conducted an unannounced visit to the facility for the purpose of following up on the submission of an Unusual Incident Report (UIR) that was received by the Regional Office on 02/07/2024. LPA met with Deseree Jones, Director, Kayla Morales, Assistant Director, and Nelioufer Jayawardana, Head of Schools/CEO to discuss the purpose of the visit. LPA observed 21 children in the Daffodil classroom with 3 staff, 18 children in the Cherry Blossom classroom with 2 staff, 20 children in the Calla Lily classroom with 2 staff, 11 children in the Sweat Pea classroom with 1 staff and 17 children in the Buttercup classroom with 1 staff.

UIR stated on 01/30/2024 at 4:45 p.m. a vendor affiliated to the facility who teaches an extracurricular activity was transitioning preschool children from outdoor to one of the preschool classroom. During that period of transition, the classroom door closed before all the children transitioned into the classroom. After the classroom door closed, the vendor discovered a preschool child standing outside of the classroom through the glass panel windows of the door. The child remained in direct sight of the vendor. At that time the vendor opened the classroom door to allow the child to transition from outdoors to the preschool classroom. UIR stated the Director and child’s parent were contacted about this incident. Along with the submitted UIR, facility administration included a copy of an electronic correspondence between the director and the child’s parent, as well as documented the date of mandatory child supervision training for all staff including the vendor.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
