13 | On August 17, 2022 Licensing Program Analyst (LPA) Zirbes conducted a follow-up complaint inspection to Ready Set Grow Learning Services. LPA met with facility representative Angie Bustillos. The purpose of the inspection was to deliver the findings for the above complaint allegation. Over the course of investigating the allegation regarding personal rights, LPA conducted confidential interviews with staff and parents. In addition, LPA completed a review of the facility records and photographic evidence of the injury. All staff denied child 1 (C1) obtained an injury on June 3, 2022. The facility records did not contain an injury report dated June 3, 2022. The infant daily report indicated that at drop off on June 3, 3022, C1’s health check was “good” however C1 had a small scratch inside the ear. The information obtained over the course of the investigation, revealed inconsistencies regarding if C1 sustained an injury to the eye while in care on June 3, 2022. Therefore, the allegation was deemed Unsubstantiated at this time. A finding of unsubstantiated means that the allegation may have happened or is valid, but there is not a preponderance of the evidence to prove that the alleged violation occurred. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Angie Bustillos.
