On February 20, 2025, at 09:40 a.m., Licensing Program Analysts (LPA) Zirbes and Cha met with Facility Representative, Zaray Ochoa. The purpose of the inspection was to conduct an unannounced annual/random inspection. During the inspection, LPA toured and inspected the Infant program in accordance with the facility sketch. LPA observed a total of five infants, with two teachers providing supervision at the time of the inspection. In addition there were four additional Administrative/Supportive staff on site. Based on LPAs observation the facility was meeting the capacity/ratio requirements during this inspection.
The center operates out of a one story building. The infant program consists of one classroom (#2), and an additional room for food preparation/diaper changing and an outdoor activity space. This center has a preschool program (364844266) and school age program (364844265). All three components utilize the following spaces: kitchen, staff office, staff break room, staff restroom, various storage areas and a garage. The program operates Monday thru Friday 6:30am -6:00pm. There is a waiver on file for potty training toddlers to use the Preschool bathroom.
Indoor/Children's Area: Indoor activity space for infants is physically separate from space used by children in the preschool and school-age child care center components. Infant center is equipped with appropriate furniture and equipment, changing table and feeding table. Furniture is maintained in good repair and safe condition (age appropriate). LPA did not observe sharp points or edges, or small parts that could be swallowed. Per staff interviews equipment, toys and furniture are sanitized daily. There is one hand washing sink within arms length of the changing table. No infant is left without supervision (visual), nor left unattended. Used diapers are put in a diaper genie. Each infant has container labeled with their name for the storage of personal items. LPA observed the cleaning compounds were stored on a high inaccessible shelf in adjoining room to the infant classroom. Cleaning compounds are also stored in a locked cabinet near the kitchen. The infant center does not have a fire place.

The building has central heating and window air conditioning units. The room temperature was comfortable at the time of this inspection. LPA observed a carbon monoxide detector in the Center. There is a working telephone on the premises. There is an isolation room for ill children. LPA reviewed the sign in and out records. Based on LPA review on this date, the sign in/out sheets were current and accurate.
No smoking is allowed on the premises. LPA did not observe baby walkers or other prohibited play equipment on the premises. Per Facility representative zero firearms or weapons are stored on the property. In addition, per facility representative there are no bodies of water on the property.
Napping: LPA observed cribs and cots. LPA observed the crib mattresses were made of a cleanable vinyl material and were in good repair. The sleeping area is separated from the infant activity space. Sheets are cleaned and changed everyday or more often as necessary, according to information received from facility representative Ochoa. LPA advised the facility representative to document the child's ability to climb out of the crib, within the children's records. A copy of regulation 101439.1 was provided during this inspection.
Other: Electrical outlets were inaccessible at the time of this inspection. LPA did not observed recalled and/ or prohibited toys or sleep/play equipment observed on the premises.
Restrooms: At this time all infants are in diapers the infant area does not have a bathroom. A waiver is on file for regarding the restrooms space for the infant license. A diaper changing area was located in the adjoining room to the classroom. The area was clean and sanitary during this inspection. LPA and facility representative discussed supervision requirements while staff are completing diaper changes.
Outdoor: Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. LPA observed the infant outdoor activity space was fully enclosed with a chain link fence. LPAs observed a piece of 2x4 wood at the base of the chain link fence. The 2x4 wood was rough to the touch. LPAs observed approximately 12 foam mats covering the cement ground. LPAs observed evidence that pre-existing foam mats were present in the space covering the cement. Per conversation with facility representative Ochoa, the recent winds damaged the foam mats and they were removed for the space. Facility representative Ochoa stated due to the extreme weather the infants have not been participating in outdoor activities. LPA advised the Center that additional floor cushioning should be provided to prevent injuries to the infant in care, and should be in place at all times. During this inspection, LPA was provided with documentation showing additional foam mats were purchased by the Administrator.

Infant File and Staff File Review: LPA reviewed a total of seven infant files and four staff files. Per LPA review, child and staff files were complete and contained all required information.

Facility Records


LPA and Facility representative reviewed the LIC 311A Records To Be Maintained At The Facility.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for
lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-
CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP.
Postings:
The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster, car seat law (PUB 269), emergency disaster plan.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated..

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/

safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Based on LPA record review and observations, one technical violation was issued today.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Zaray Ochoa. Appeal Rights were provided.
